Description

Searching for Service Disabled Veteran Owned Small Businesses and Veteran Owned Small Businesses using NAICS 621999 and provides Mobile Lithotripsy Services. The contractor shall provide Mobile Lithotripsy Services, Extracorporeal Shock Wave Lithotripsy (ESWL) equipment and related technical support (minimum of two (2) lithotripsy specialists) at the VA Medical Center, Alexandria, LA, two (2) Wednesday afternoons per month for an estimated 48 patients per year. The VA estimates that there will be 1 to 3 patients per visit. The procedure(s) shall include single stone and multiple stones procedures per unilateral or bilateral episode. The equipment must be prepared to perform the first procedure of the day at a mutually agreed upon time and continue to perform procedures until the VAMC's schedule for the day is complete Request the following questions be answered and returned to the contracting officer by 4:00PM EST on 10 June 2016. 1.Are you a Service Disabled Veteran Owned Small Business or a Veteran Owned Small Business registered at VETBIZ at website https://www.vip.vetbiz.gov/Public/Search/ViewSearchResults.aspx?SCID=3030816? 2.Can you provide documentation of your registration? 3.When does your current registration expire? 4.Do you provide mobile Extracorporeal Shock Wave Lithotripsy (ESWL) services to include equipment and technicians to operate? 5.Do you provide this service in Louisiana? 6.Are the technicians licensed and certified to perform services in the state of Louisiana? 7.Can you provide documentation of licenses and/or certifications for technicians? 8.What is the minimum number of EWSL procedures per month required to issue a contract? 9.How much advance notice do you require in the event of cancellation by the Government?
