Description
This is a request for information (RFI) to identify potential sources for Patient Transportation Services to serve the Join Base San Antonio Military Medical Center (SAMMC), Fort Sam Houston TX and Camp Bullis area. This is for Emergent and Non-Emergent (Basic Life Support and Advance Life Support) twenty four (24) hour 7 days a week ambulance Service to transport beneficiaries to and from San Antonio surrounding areas. The purpose of this RFI is to gain industry feedback and to gain knowledge of the interest, capabilities, and qualifications of potential sources. The Government is not soliciting offers at this time and a solicitation is not currently available. In accordance with FAR 15.201, this sources sought announcement is not to be construed as a commitment by the Government, implied or otherwise, to issue a solicitation or to award a contract. No reimbursement will be made for any costs associated with providing information in response to this synopsis or any follow-up information requests. Responses to this request will be considered for the purpose of determining how the requirements will be stated and how the procurement will be conducted. Respondents should clearly describe their capabilities to include management experience, equipment and personnel or the ability to obtain them. Furthermore, interested sources must also meet all requirements of Federal, State, and Local Municipal codes regarding operations of this type service. The intended procurement will be classified under North American Industrial Classification (NAICS) code 621910 with a size standard of $15M. Interested firms responding to this market survey must provide a capability statement demonstrating their experience, skills and capability to fulfill the Government's requirements for the above. Submissions shall be in sufficient enough detail, but not to exceed a total of 3 pages, so that the Government can determine the experience and capability of your firm to provide the requirements above. Your capability statement, not to exceed 3 pages. POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: A brief capability statement demonstrating past performance. Capability Statement shall not exceed more than 3 pages. 1. Company name and address, point of contact with phone number, and DUNS number. 2. Company socio-economic status to identify large and small business, as well as any other socio-economic classifications (e.g., 8(a), HUBZone, etc.). 3. Brief narrative of capability of performance Set-Asides - At this time the government is requesting information in order to provide a final determination related to set aside. Upon review of the market research and coordination of appropriate parties a determination will be made The response date to this RFI is June 10, 2016 at 12 noon CST. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. Any organization responding to this notice should ensure that its response is complete and sufficiently detailed to allow the Government to determine the organization's qualifications to perform the work. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. After a review of the responses received, a pre-solicitation synopsis and solicitation may be published in Federal Business Opportunities. However, responses to this notice will not be considered adequate response to a solicitation. The Government reserves the right to use any non-proprietary technical information in any resultant solicitation(s). Respondents WILL NOT be notified of the results of the market research. DO NOT include Proprietary, classified, confidential, or sensitive information in responses.
