- Amendment 0001 is hereby issued as indicated in the attached PDF document. AMENDMENT 0001: Amendment 0001 is hereby issued as indicated in the attached PDF document. The proposal due date is extended through June 15, 2016 @ 1:00 p.m. EST. __________________________________________ The U.S. Consumer Product Safety Commission (CPSC) intends to procure Human Factors Research Design and Support Services for a performance period one Base year and Four twelve-month option periods. This notice is a combined synopsis/solicitation for commercial services in accordance with Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This is a firm-fixed price IDIQ type contract and will be solicited as a Request for Quotation (RFQ) CPSC-Q-16-0031 for Human Factors Research Design and Support Services as stated in the attached performance work statement. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. The following provisions and clauses are enclosed in the solicitation attachment: 52.204-10, 52.212-1, 52.212-4, 52.212-5, 52.216-8, 52.216-19, 52.216-22, 52.217-8, 52.217-9, 52.227-14, and 52.232-40. Project Overview: The Contractor shall independently, not as an agent of the Government, perform any of the following: 1) The Contractor shall design a user research study that may involve methods including, but not limited to, focus groups, surveys, interviews, observational research, human factors research, or usability evaluations. 2) For research involving interventions with human subjects that involve more than minimal risk, the Contractor shall prepare all necessary documents for the Institutional Review Board ("IRB"). 3) The Contractor shall implement all activities needed to secure and obtain the Institutional Review Board ("IRB") approval for research studies involving interventions with human subjects that involve more than minimal risk. 4) The Contractor, with the assistance of CPSC, shall prepare all necessary documents and implement any changes to the study plan needed for Office of Management and Budget ("OMB") Paperwork Reduction Act ("PRA") approval. 5) The Contractor shall recruit study participants that are representative of the target user population. 6) The Contractor shall secure a location for the study that is appropriate for the user research method. 7) The Contractor shall obtain appropriate data collection tools needed for the study. 8) The Contractor shall conduct the study and collect data according to the study design. 9) The Contractor shall generate a report detailing all results gathered as a result of the study. All responsible sources may submit an offer which shall be considered by the agency. The Government anticipates that the requirement will fall under NAICS 541910 code and small business sized standard is $15 million. It is the responsibility of the interested parties to monitor FedBizzOpps for any amendments to this request for quotation. ALL QUOTES MUST BE VALID THROUGH September 30, 2016. This acquisition is being conducted under FAR Part 12 in conjunction with FAR 13 Simplified Acquisition Procedures (13.5). The date and time for submission of quotes and any supporting documentation shall be received no later than June 9, 2016, 9:00 am ET. All questions and/or clarifications in response to the solicitation shall be e-mailed to pfolts@cpsc.gov no later than May 25, 2016, 12:00 pm ET. No telephone calls please. If you are interested in this award, please complete the attached RFQ as follows: 1) Cover sheet (standard form 1449) - Block 17 with the contractor's name, DUNS number, address and telephone number must be complete. The signature, name and title of an individual authorized to commit the company must complete blocks 30a, b and c of the Standard Form 1449. 2) All offeors must have a Tax Identification number and be registered with the System for Award Management (SAM / www.SAM.gov) in order to be eligible for award of this contract. 3) Please review the Statement of Work found on pages 4 - 7. 4) Review the Additional Information to Offerors section (pages 39 - 45). This portion of the RFQ will also provide instruction on submitting your quotation.
