- Attachment 4 Statement of Work Attachment 3 Past Performance Check Attachment 2 Vendor Enrollment Form Attachment 1 Nondisclosure Form Solicitation - Request for Proposals The United States Capitol Police Background Investigations Section requires contractor assistance in conducting pre-employment polygraph examinations on selected sworn and civilian applicants in accordance with USCP practices and procedures. The unique mission of the Department of protecting the United States Congress and those who work and visit the congressional complex on a daily basis, makes it imperative for the Department to hire only those applicants that are above reproach. The Background Investigations Section is responsible for conducting background investigations on potential employees to ensure that the Department hires only the best-qualified applicants, into positions of trust. The polygraph examination is an integral component of our investigative process to ensure the integrity of the Department and the safety of the Congressional Community. Accordingly, the applicant must possess the honesty and integrity that are needed to successfully perform the duties of a Law Enforcement Officer. The Contractor shall be able to provide certification confirming each Polygraph Examiner has graduated from the "Psychophysiological Detection of Deception (PPD) program and received their certification from the National Center for Creditability Assessment (NCCA) or its predecessor institutions. The following predecessors institutions are acceptable: a. U.S. Military Policy School/Army Polygraph School (USAMPS) b. Department of Defense Polygraph Institute (DODPI) c. Defense Academy for Credibility Assessment (DACA) The Contractor shall provide documentation of such license with the proposal. All licenses shall be current and valid during the term of the contract for any and all examiners to be utilized. The USCP estimates that the Contractor shall be able to accommodate and administer up to 80 polygraph examinations per month at the USCP examination facility. The Solicitation for this requirement is expected to be issued 6/3/2016. Submission of Offers - Dates: One (1) Original and (4) Copies of Proposals must be hand delivered to: USCP Office of Procurement Timothy Stinson 499 South Capitol Street SW Washington, DC 20003 Facsimile and e-mail proposals are not authorized. All proposals submitted must include reference to USCP RFP Number QSU OHRR201600001 Polygraph Services. The offeror's proposal must include the offeror's name, address, phone number and the name, title, phone and facsimile numbers of persons authorized to negotiate on the offeror's behalf. The offeror's proposal shall state the following items: DUNS number, taxpayer identification number (TIN), and CAGE code. The name of the official authorized to bind the offeror's company shall be clearly identified and if an amendment(s) is issued, the offeror must acknowledge acceptance of the amendment. Only complete packages on or before 3:00 PM Eastern Time on June 28, 2016 will be considered. To allow for sufficient time to address questions, all questions must be submitted via email no later than 12:00 PM Eastern Time on June 10, 2016. Response to the questions will be posted on FedBizzOpps.gov via RFP amendment no later than June 17, 2016. When submitting questions, the Contractor shall reference the area of the RFP where the question is derived. NOTICE: The Offeror's Technical and Price proposal must be submitted in separate documents.
