- PROJECT: VA HOME BASED PRIMARY CARE SURVEY BACKGROUND: Since 1995 variants of the Survey of Healthcare Experiences of Patients (SHEP) program have focused on two broad based patient populations; recently discharged inpatients, and patients receiving recent VA ambulatory care. The SHEP program has also surveyed special populations like Operation Iraqi Freedom (OIF)/Operation Enduring Freedom (OEF)/Operation New Dawn (OND) patients, Home Based Primary Care (HBPC), and others. The SHEP program has consistently measured and reported results at various levels of the organization and to other internal and external stakeholders. PURPOSE: The purpose of this requirement is for the VHA Office of Geriatrics and Extended Care (GEC) to obtain a contractor with provisional or a full Home Health Consumer Assessment of Health Care Providers and Systems (HHCAHPS) certification capable of providing mail-out administration of Veteran patient experience surveys and related services to Veterans Home Based Primary Care (HBPC) at or through all 157 Medical Centers and approximately 1,000 Veterans Health Administration (VHA) Outpatient Clinics. This includes Puerto Rico and surrounding coverage areas, and the Philippines, unless otherwise specified. The principal patient populations to be surveyed are patients enrolled in the Home Based Primary Care program for a minimum 30 days, but surveys for other populations may be developed and fielded, as necessary. Provisional or full certification must be maintained or improved upon, (i.e. provisional to full certification), and maintained for the life of the contract. The estimated total number of core surveys is approximately 40,000 per year. The Survey Preparation phase shall involve intense collaboration between GEC and the contractor as questionnaires are designed, all materials for their mailing are set-up and proofed, samples are drawn, bad addresses are eliminated, and final sample sizes are determined. Completing active data collection, providing regular reports of progress, and delivery of accurate final data sets, results reports, and related files shall be the prime responsibility of the contractor. All HHCAHPS based surveys administered shall be fielded, collected, analyzed, and reported using established definitions as specified by HHCAHPS, AHRQ, NCQA, other potential accrediting bodies or survey sponsors, or VHA as appropriate for the specific patient population or survey being administered. The results are reported to the Office of Geriatrics and Extended Care. Composite/Reporting Measures reports shall be appropriately weighted and risk adjusted according to GEC's specifications. The contractor shall develop full national, VISN, facility, and clinic-level reporting as required by each report. The contractor shall also provide technical web-based training, education and documentation to GEC and other VHA field staff as needed to insure maximum practical use at all organizational levels. The training and educational materials can be recorded for reuse and orientation for new staff or for refresher training. INFORMATION REQUESTED: The contractor should provide supportive documentation that describes past projects and current capabilities that are of similar size, scope, and complexity. Furnished references from past performance is highly encouraged and should include contractor or delivery order number, type of contract (i.e., FPP), the customer's point of contact information (name/phone/email, position), procuring contracting officer's email address, if the RFI responder performed as prime or subcontractor, price/cost of contract. SPECIFIC QUESTIONS TO BE ADDRESSED (all questions listed below must answered completely): 1.Describe your capacity/capability to provide/perform HHCAHPS survey requirements for VA patients to support healthcare programs in similar size, scope and complexity as described in the attached draft PWS? 2.Identify and describe your perceived risk with the complexity of this requirement. 3.The Government has determined the North American Industry Classification System (NAICS) code 541611-- Administrative Management and General Management Consulting Services is the most appropriate for this require. If you believe another code would be more appropriate, please list the code and justification. 4.Identify the estimated number of full time equivalent employees, labor categories, and any other relevant pricing/effort components/industry standards to consider, that support the tasks as described in the attached draft PWS. Include a rationale/justification for an alternate contract type, if any. Responses: Responses should be single-spaced, 12 point-font, with numbered pages and should not exceed 12 pages in length, including cover page, appendices, tables and figures. Responses should use the following subject heading: "RFI - VA Home Based Primary Care Survey". Comments on the PWS are not requested. The Government will not pay for information solicited and will not recognize or reimburse any cost associated with any RFI submission. Therefore, the Government recommends vendors submit electronic version of, or web-links to previously prepared presentations, documentation, white papers, and other relevant information.
