Description

COMBINED SYNOPSIS/SOLICITATION Valve Replacement at Michael E. DeBakey Veterans Affairs Medical Center in Houston, Texas (I) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice and in accordance with the simplified acquisition procedures authorized in FAR Part 13. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (II) This solicitation is issued as a request for quotation (RFQ). Submit written quotes on RFQ Number: VA256-16-Q-0757. (III) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-87. (IV) This solicitation is being issued as a Total Service Disabled Veterans Owned Small Business Set-Aside. The associated NAICS code is 238220, Plumbing, Heating, and Air-Conditioning Contractors. The small business size standard is $15.0 Million. (V) This combined solicitation/synopsis is for purchase of the following commercial services: Contractor shall furnish all labor, elevator equipment, all necessary skilled and certified labor for the original equipment manufacturer, materials, parts, software, transportation, travel, and supervision to provide Valve Replacement services at the Michael E. DeBakey VA Medical Center, 2002 Holcombe Boulevard, Houston, Texas 77030. (VI) Description of requirements is as follows: Contractor shall perform close and lock-out of isolation valves before and after mixing valves. Disconnect pneumatic tubing from existing valve. Drain water from valve and assembly. Remove faulty valves. Install new NELES R-Series Segmented Valve Flagged RE type Wafer RE1 type with CV trims for general application to noise/cavitation Q-trim for reducing aerodynamic noise/preventing cavitation/ ND9000 intelligent valve controller for precise control, and performance monitoring or equivalent with pneumatic actuator. Re-install existing pneumatic tubing. Calibrate and verify correct actuator operation. Leak test work area. Install new Phenolic type insulation with blue PVC pipe covering. Remove lock-outs and place system back in normal operation. Provide supervision of installation. Provide lifts and or ladders to safely access work area. Program new valves into Building Automation System (BAS) etc. In addition the vendor shall provide an on-site visit to verify accessibility, system integrity, verify conductivity, pipe size, control scheme, and replace of defective components. See attached Statement of Work which applies to the one time performance of Valve Replacement with a Tentative award date of 06/29/2016 and completion date before 09/30/2016. The project will be completed in accordance with the Department of Labor Wage Rates: WD 2015-5233, Revision No. 3, dated 01/19/2015, which can be found on: www.wdol.gov
