Description
PWS This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. Solicitation number W9114F-16-T-0068 is issued as a request for quotation (RFQ). Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-50, effective 16 March 2011. Not a set-aside. North American Industry Classification System (NAICS) code for this solicitation is 813920. The Regional Health Contracting Office- Europe has a requirement to procure non-personal services to perform Trauma Certification Survey for verification of Landstuhl Regional Medical Center, (LRMC) compliance with ACS Standards as Level III Trauma Center, in accordance with the American College of Surgeons Committee on Trauma Verification. The contract line items (CLINs) for this acquisition are as follows: Line Items 0001 QTY 1 JOB Non-Personal services for two (2) ACS Surveyors to perform On-Site Trauma Certification Survey for verification of Landstuhl Regional Medical Center, (LRMC) compliance with ACS Standards as Level III Trauma Center, in accordance with the enclosed Performance Work Statement (PWS). Level III Trauma Center certification shall have three (3) years validity from 1 July 2017 thru 30 June 2019. Period of Performance: 1-30 April 2017 Line Item 0002 QTY 1 JOB Not-to-exceed (NTE) reimbursement of costs for all travel, transportation, meals, lodging, and incidentals IAW FAR Part 31 and IAW the rates prescribed by the Joint Travel Regulations (JTR) and § 1.8.1 of the PWS. Costs for these expenses will be reviewed, certified and approved by the Contracting Officer's Representative (COR). All travel and transportation shall utilize commercial sources and carriers provided the method used for the appropriate geographical area results in reasonable charges to the government. The Government will not pay for business class or first-class travel. Period of Performance: April 2017 Line Item 0003 QTY 1 Contractor Manpower Reporting (CMR) In accordance with PWS paragraph 6. entitled "Accounting for Contract Services/Contractor Manpower Reporting". Paragraph 52.212-1(b)(4) is hereby replaced with the following: FACTOR 1: TECHNICAL. Factor I - Technical. The vendor's quote shall demonstrate the capability to complete the on-site survey and to provide Trauma Certification and verification on behalf of the American College of Surgeons (ACS) to include a comprehensive report of strengths, weaknesses and recommendation and successful verification of LRMC as a Level III Trauma Center with a three (3) years validity. Failure to receive an Acceptable rating of any of the above salient characteristics / evaluation criteria will result in an Unacceptable technical rating unless the error(s), deficiency (ies) or omission (s) can be corrected. FACTOR II: PRICE. Quoters shall submit prices on the price schedule in the solicitation. The Government will select the quote that is based upon the lowest evaluated price, technically acceptable quote. All Contract Line Item Numbers (CLINS) must be priced by the quoter. If the quoter failed to price all CLINs, the quote will not be considered for award. Paragraph 52.212-1(c) is hereby replaced with the following: The quoter agrees to hold the prices in its quotation firm for 60 calendar days from the date specified for receipt of quotations, unless another time period is specified in an addendum to the Request for Quotation. Paragraph 52.212-1(d) does not apply to this Request for Quotation. Paragraph 52.212-1(i) does not apply to this Request for Quotation. Add the following: Paragraph 52.212-1 (h)(ii) Only one Purchase Order will be awarded from this RFQ. Award will be on an "All OR NONE" basis. Quotes for less than all of the solicited line items will be rejected as nonconforming and unacceptable to the RFQ. Para (n) ELECTRONIC QUOTES. Vendors may submit electronic quotes in response to this RFQ. All quotes are due on the date Local Germany time shown on page 1, Block 8 of the Standard Form 1449 (RFQ )to the following email address: domenico.maddaloni.ln@mail.mil. Vendors may call Domenico Maddaloni at 011-49-6371-9464-5414 to verify receipt of the quote. If the vendor chooses to email the quote, the Government will not be responsible for any failure of transmission or receipt of the quote, or any failure of the vendor to verify receipt of the emailed quote. If receipt of the quote submitted by email is not verified within 24 hours of submission, the vendor must resubmit their quote by fax. Para (o) QUESTIONS. Prospective vendors are strongly encouraged to submit any questions regarding this RFQ no later than two (2) work days prior to the RFQ due date specified in block 8 of the SF 1449 cover page to domenico.maddaloni.ln@mail.mil or by fax (011) 49-6371-86-8070. If the vendor chooses to email the questions, the Government will not be responsible for any failure of transmission or receipt of the questions, or any failure of the vendor to verify receipt of the emailed inquiry. If receipt of questions submitted by email is not verified within 24 hours of submission, the vendor must resubmit their inquiry by fax. All questions received will be answered officially through an amendment for distribution to all prospective vendors. Paragraph 52.212-1 (p) The non-FAR Part 12 discretionary FAR, DFARS, AFARS, and LOCAL provisions included herein are incorporated into this Request for Quotation either by reference or in full text. If incorporated by reference, see provision 52.252-1 herein for locations where full text can be found. Paragraph 52.212-1(r) If a Quoter believes that the requirements in these instructions contain an error, omission, or are otherwise unsound, the Quoter shall immediately notify the Contracting Officer (KO) in writing with supporting rationale. ACQUISITION PROCEDURE: This is a commercial item under FAR Part 12 and the acquisition procedures to be used for this purchase will be in accordance with FAR Part 13 Simplified Acquisition Procedures. SPECIAL NOTE: All vendors must be registered in the System for Award Management (SAM) prior to award. Lack of registration shall make a quote ineligible for award. Vendors may obtain information on registration and annual confirmation requirements by calling 1-866-606-8220 or via Internet at http://www.sam.gov. ADDENDUM TO FAR 52.212-2 Evaluation. Award will be made using the lowest price technically acceptable (LPTA) evaluation process. Award will be made to the responsible Quoter on the basis of the lowest evaluated price of quotes meeting or exceeding the acceptability standards for technical factors. The Government intends to make an award without conducting discussions. After the initial evaluation of quotations is completed, if one or more quotations are rated Acceptable Technical, the initial evaluation will become the final evaluation of quotations and award will be made without discussions to the lowest priced quote. If any one (1) technical subfactor is rated Unacceptable after the initial evaluation, the technical factor will be rated Unacceptable and the quote will no longer be considered for award. If all quotations submitted result in an unacceptable technical evaluation, then clarifications may be necessary. In that event, the Government evaluation team will evaluate the Quoter's response to the Government exchanges with each Quoter. TECHNICAL EVALUATION STANDARDS: The quoter's technical quote will be evaluated using the following standards for a final rating: 1. Acceptable - A quote that meets the Government's technical requirements identified in the Request for Quotation. 2. Unacceptable - Quote A quote that fails to meet the Government's technical requirements identified in the Request for Quotation. FACTOR II: PRICE. The Government will select the quote that is based upon the lowest evaluated price, technically acceptable quote. All Contract Line Item Numbers (CLINS) must be priced by the quoter. If the quoter failed to price all CLINs, the quote will not be considered for award. (b) A quotation is not an offer and, consequently, cannot be accepted by the Government to form a binding contract. Therefore, issuance by the Government of an order in response to a supplier's quotation does not establish a contract. The order is an offer by the Government to the supplier to buy certain supplies or services upon specified terms and conditions. A contract is established when the supplier accepts the offer. (c) The word "quoter" shall be substituted for the word "offeror" wherever it appears in FAR 52.212-3, Alternate I and DFARS 252.212-7000.
