- Past Performance Contacts Pricing Spreadsheet HSBP1016Q0104 includes the statement of work This procurement is for Farrier Services in accordance with FAR Subpart 13, the request for quotation (RFQ) and the statement of work (SOW). Supplier quotations will be provided in two separate documents submitted via www.fbo.gov or directly to kirk.w.hunt@cbp.dhs.gov. Quotation documents must be compatible with Microsoft Word or Excel or viewable in Adobe Acrobat. Documents must not exceed 5 pages (8.5" x 11" letter-size paper) or 5 megabytes in total file size. The two parts, as separate documents, are: 1) Business Quotation a) Unit Price using the provided spreadsheet (inclusive of delivery and all other fees and charges) and b) Service availability (days after award), plus 2) Technical Quotation a) Service Description to include pertinent technical details. b) Explanation of how the Farrier Services being offered meets all of the requirements of the SOW. c) A statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. d) No more than three (3) Customer Contacts from similar, recent contracts (per provided form). The prior contracts should be for work similar to this requirement. Prior contracts should be ongoing or completed within thirty-six (36) months of the date of submission of quotes. The Government intends to evaluate quotations and award without discussions with offerors (except clarifications as described in FAR). Therefore, the offeror's initial quotation should contain the offeror's best price and service availability; provide full and complete service descriptions; fully explain how the offeror intends to meet all the requirements of the SOW; a statement regarding agreement with terms, conditions and provisions; and provide complete contact data for no more than three (3) recent Customers. Failure to provide complete information in accordance with FAR 52.212-1, Instructions to Offerors -- Commercial Items; may cause an offer to be excluded from award. The Government reserves the right to conduct discussions if the Contracting Officer determines them to be necessary. Award will be made on the basis of best value: The offer that provides the greatest overall benefit, as determined by the Government, in response to the requirement. While price will be considered, the lowest priced offer may, or may not, receive award against this solicitation. While technical merit will be considered, the most highly rated offer may, or may not, receive award against this solicitation. PERIOD OF PERFORMANCE Base Year: July 1, 2016 through June 30, 2017. Option Year One: July 1, 2017 through June 30, 2018. Option Year Two: July 1, 2018 through June 30, 2019. Option Year Three: July 1, 2019 through June 30, 2020. Option Year Four: July 1, 2020 through June 30, 2021.
