Please quote 1 EA or no quote on each line item FAR Clauses Reps & Certs RFQ for vendors fill out page 1 Solicitation for Indian Owned Businesses Requestor is interested in establishing a BPA for various medical supplies. This is a 100% Buy Indian Set Asidefor American indian/Alaskan Native Owned Firms who must also be a small business Vendor must provide documentationin in accordance with the requirements under the Buy Indian Act (48CFR CH.3) (10-1-10 Edition). Please provide documentation for proof of at least 51% indian owned with your BPA package. Vendor is to e-mail package to Christopher.Millard@IHS.gov. Attached is a list of medical supplies to count as pricing document. Please quote 1 EA for pricing, or put no quote on items you cannot price. Please submit formal quote with vendor information, and price. If vendor has a file of medical supplies and, pricing list for all medical items on hand please send with your BPA information via e-mail
