Description

Special Notice for Sole Source Award for DTRAX Facet Spinal Fusion Implants Pursuant to FAR Subpart 13.106-1(b) (1), the U.S. Army, MEDCOM, Regional Health Contracting Office-Pacific, Joint Base Lewis-McChord, Health Contracting Cell, Lacey WA, intends to issue a sole source award under solicitation W81K02-16-T-0146 to Providence Medical Technologies, Inc., in the amount of $36,000.00, for DTRAX Facet Spinal Fusion Implants to be used in an urgent surgical procedure for a specific patient as required by the Surgeon from the Department of Anesthesia and Operative Services (DOAS) at Madigan Army Medical Center (MAMC), Tacoma WA. This notice is not a request for proposal and a solicitation will not be issued. Any vendor that believes it can provide the stated services within the above parameters as indicated can submit supporting documentation demonstrating their service capability and specifications. Responses received will be used for future acquisitions. Information can be sent to the Purchasing Agent; Mr. Walter J. Bischoff, email; walter.j.bischoff.civ@mail.mil. No telephone or facsimile requests will be accepted. This procurement will be prepared in accordance with FAR Part 13, using Simplified Acquisition Procedures.
