- Salient Characteristics Regional Health Contracting Office-Atlantic-Provisional- Fort Gordon Cell W91YTZ-16-T-0216 This is a combined synopsis/solicitation for commercial equipment prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is W91YTZ-16-T-0216. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72. The North American Industry Classification System Code (NAICS) for this procurement is 339112 - Surgical and Medical Instrument. This will be an unrestricted requirement. The Regional Health Contracting Office-(Atlantic)-(Provisional) -Fort Gordon Cell (RHCO-(A)(P) -FGC) Fort Gordon, GA has a requirement for 1 each Plethysmograph, Segmental (SS-FC10 FloChec Digital ABI) Brand Name or Equal for support of Blanchfield Army Community Hospital, Fort Campbell, KY. ITEM NO. Price must include shipping and handling charges. Inclusive of the grand total, the Offeror will provide complete installation of equipment. Offeror must be registered in the SAM database before an award can be made. If the offeror is not registered in the SAM database, it may do so through the SAM website at http://www.SAM.gov. The selected Offeror must comply with the following commercial service terms and the provisions at FAR 52.212-1, Instructions to Offerors-Commercial (APR 2014), apply to this acquisition. 52.212-1, Addendum.
