Description
This is an updated SOW. This is a diagram of the CONNEX layout. Please find attached an updated SOW and Movement Diagram. •1.0 INTRODUCTION The Naval Surface Warfare Center (NSWC), Corona Division, Property Management Division, Code 00M2 requires crane support for relocation of container boxes throughout NSWC Corona. •2.0 BACKGROUND The Naval Surface Warfare Center (NSWC), Corona Division, Property Management Division, Code 00M has a requirement to establish a centralize storage area for plant property and project stock. •3.0 SPECIFICATIONS Requesting crane services in order to reposition containers at different 3 locations within the NWS Seal Beach, Det. Norco. Requirements include: (a) 90-120 ton hydraulic crane with operator (b) 3, 40 ft flatbed truck with qualified drivers (c) Qualified riggers (d) Qualified personnel to handle tag lines throughout the process A 90-120 ton hydraulic crane and 40ft flatbed truck will be utilized to reposition 7 containers from location 1 and 4 containers from location 2 to one centralized location near the 4 th street gate entrance (location 3). Location 1 is approximately 250 ft from the centralized location, while location 2 is approximately 0.5 miles from the centralized location. 10 additional containers will be moved within location 3 for a total of 19 containers. Inspections and Clearance Prior to arrival, the following requirements must be completed and approved by Naval Facilities Engineering Command (NAVFAC) Southwest: 1. Contractor will submit a 7 day advanced notice prior to any crane coming onboard Seal Beach Det. Norco. 2. Must have the CERTIFICATE OF COMPLIANCE FORM filled out and given to the Crain inspector 48 hour prior to the cranes arrival. 3. Will have the following forms filled out upon the crane arrival at Seal Beach Det. Norco, or Email mail them a week prior to the crane arrival, to be presented to the crane inspector a. CONTRACTOR CRANE COMPLIANCE REVIEW FORM b. CONTRACTOR RIGGING GEAR OVERSIGHT GUIDELINES c. CONTRACTOR CRANE OPERATION CHECKLIST 4. COMPLEX LIFT FORM will be filled out as per the EM 385, Definition of a non-routine crane lift requiring detail planning and additional or unusual safety precautions. 5. All other equipment as per the CONTRACTOR MULTI PURPOSE MACHINE REFERENCE form shall be filled out before any equipment brought on or used on Government property or projects. Upon arrival, NAVFAC will conduct a crane inspection to determine if the crane is cleared for operations on NWS Seal Beach, Det. Norco. Once cleared, NAVFAC will inspect the crane setup at the first setup location and every subsequent setup location to ensure compliance with all Navy safety guidelines and regulations. Safety All personnel involved in crane operations on NWS Seal Beach, Det. Norco must be familiar with the following Navy instructions and regulations: 1. NAVFAC NAVY CRANE CENTER P-307 2. EM 385-1-1, Safety and Health Requirements Manual Section 16 Use of Weight Handling Equipment (WHE) a. The WHE must be inspected and approved by the Government Representative prior to performing any lifts. An Operating Permit will be issued upon completion. c. The WHE shall meet as a minimum, the requirements of OSHA using the guidance of 29 CFR 1910.180, 29 CFR 1926, and Army Corps of Engineer's Publication EM 385-1-1, Safety and Health Requirements Manual. If long shoring is involved, include 29 CFR 1918. d. A copy of the Crane Certificate of Compliance and operating permit shall be posted in the crane's cab. e. The crane operator shall have their license on their person while performing crane operations. f. Provide any other pertinent documentation. g. The contractor may be subject to spot checks of crane operations by the Government Representative. Accident Notification and Reports 1. For recordable injuries and illnesses, and property damage accidents resulting in at least $2,000 in damages, the Prime Contractor shall conduct an accident investigation to establish the root cause(s) of the accident fill out the Contractor Incident Report (CIR) electronically and submit via the NAVFAC Enterprise Safety Applications Management System (ESAMS) within 5 calendar days. The Contracting Officer will provide copies of any required or special forms. 2. For any weight handling equipment accident (including rigging gear accidents), the Prime Contractor shall conduct an accident investigation to establish the root cause(s) of the accident, complete the WHE Accident Report (Crane and Rigging Gear) form and provide the report to the Contracting Officer within 30 calendar days of the accident. Crane operations shall not proceed until cause is determined and corrective actions have been implemented to the satisfaction of the contracting officer. The Contracting Officer will provide a blank copy of the accident report form. 3. Notify the Contracting Officer as soon as practical, but not later than four hours, after any accident meeting the definition of Recordable Injuries or Illnesses or High Visibility Accidents, property damage equal to or greater than $2,000, or any weight handling equipment accident. Include contractor name; contract title; type of contract; name of activity, installation or location where accident occurred; date and time of accident; names of personnel injured; extent of property damage, if any; extent of injury, if known, and brief description of accident ( e.g., type of construction equipment used, PPE used). Preserve the conditions and evidence on accident site until the Government investigation team arrives and Government investigation is conducted. 4.0 REQUIRED DELIVERY DATE: TBD 5.0 SHIPPING ADDRESS: NSWC Corona Division Attn: TBD 1999 Fourth Street Corona, CA 92860-5000
