Description
Draft Performance Work Statement THIS IS A REQUEST FOR INFORMATION (RFI)/SOURCES SOUGHT (SS) NOTICE ONLY; it is not a Solicitation (i.e. Request for Proposal, Request for Quotation, or Invitation for Bids) This RFI is part of a Government market research effort to determine the scope of industry capabilities and interest and will be treated as information only. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responses to this RFI are strictly voluntary and the Government will not pay respondents for information provided in response to this RFI. Responses to this RFI will not be returned and respondents will not be notified of the result of the review. SPECIAL NOTICE: The Health Readiness Contracting Office Provisional (HRCO{P}) intends to award a short term contract for Licensed Vocational Nursing Services. It is anticipated that the indefinite delivery, indefinite quantity contract will have a period of performance from October 1, 2016 through March 31, 2017; and the contract will include FAR Clause 52.217-8, Option to Extend Services. DESCRIPTION OF REQUIREMENT: The U.S. Army Medical Command has a requirement to obtain personal services of licensed vocational nurses (LVNs) and certified nurse assistants (CNAs) for the San Antonio Military Health System (SAMHS), which is comprised of the Brook Army Medical Center (BAMC), Fort Sam Houston, Texas; Wilford Hall Ambulatory Surgical Center (WHASC), formerly Wilford Hall Medical Center (WHMC), Lackland Air Force Base, Texas; and their outlying clinics throughout the San Antonio, Texas area. The scope of services provided at SAMHS covers the same range of services as those that are provided in civilian hospitals of similar size and complexity. The SAMHS provides a broad scope of services including a level one trauma center, the burn unit, Center for the Intrepid, multiple critical care units, intensive care units, step down units, medical-surgical units, pediatrics, maternal child, surgical services, extensive specialty/subspecialty services supported by Graduate Medical Education program, ambulatory clinics, and other specialty clinics. The contractor shall provide employee support for a seven (7) day/week, 24 hour/day operation, to include holidays. All contract healthcare providers shall perform essentially the same functions, within the scope of acceptable practice for the specific specialty or position, as those required by the military treatment facilities government service healthcare professionals of similar experience and in similar duty assignments. The contractor shall provide profile application packets on each employee who will be performing services under the contract no later than 10 business days prior to the start of work. The profile application packets shall provide the following information: (a) Completed Centralized Credentials Quality Assurance System (CCQAS) data entry form; (b) primary source verification of state licenses with expiration date, license number, name and contact information for individual performing primary source verification attesting to direct state verification and the method of verification; (c) contractor conducted Criminal History Background Check; (d) education history with primary source verification of college attended/degree; (e) Certifications: Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), Trauma Nursing Core Course (TNCC), and any professional certifications required for the position; (f) employment history and resume; (g) immunization status and health examination date; and (h) skills assessment checklists, competency assessments, medication examinations, telemetry examinations, or any other skills verification necessary for specialty and location (specifics will be outlined in the task orders. QUESTIONS FOR INDUSTRY: The Government desires that respondents offer their capabilities on the following questions related to the requirement. 1. What are the customary commercial practices for providing these types of services for non-governmental entities, e.g., contract type, pricing structure, task order driven contract, etc.? 2. Does your company currently have the capability to recruit approximately 240 licensed vocational nurses (LVNs)/certified nurse assistants (CNAs) on short term notice for services to begin uninterrupted on October 1, 2016? Describe how your company possesses this capability. 3. Does your company currently have the capability to perform primary source verification and background checks for approximately 240 licensed vocational nurses (LVNs)/certified nurse assistants (CNAs); and submit profile application packets for each individual on short term notice for services to begin uninterrupted on October 1, 2016? Describe how your company possesses this capability. 4. Based on your experience, what is the most cost effective contract type and payment method (i.e., monthly firm fixed-price, separate price per service (price list), cost reimbursement, or combination, etc.)? 5. Does your firm have past performance providing Personal Licensed Vocational Nurse (LVN) and Certified Nurse Assistants (CNAs) Services in this PWS at multiple geographically dispersed locations of similar magnitude? If so, please provide the following information for each relevant contract: (1) name of your customer; (2) contract number; (3) contract start date; (4) contract length; (5) total dollar value for all periods; (6) place of performance; (7) type of labor categories; and (8) a brief description of services? 6. For NAICS 621399 - Nurses, Licensed practical or registered, offices (e.g.centers, clinics) small business size standard $7,500,000.00 is your company considered a small business? If your company is a small business, do you fall under any small business subcategory (SDB, SDVOSB, Hub Zoned, 8a, etc)? If you are an 8a eligible company, please provide current status regarding your scheduled graduation date from the 8a program under this NAICS code. 7. Provide any concerns, comments or recommendations you may have with the attached draft PWS. Give specific PWS paragraph numbers for reference. RESPONSES: Respondents to this RFI/SS Notice are to describe their interest and ability to perform the requirements summarized within this notice. Responses are to contain (1) company name, (2) CAGE code, (3) primary point of contact information, to include telephone number and email address. Responses should be formatted as either MS Word (.doc) or Adobe Portable Document Format (.pdf) and should be limited to a maximum of two (2) pages. Proprietary/Competition Sensitive information (appropriately marked) will be protected from disclosure to the greatest extent practical, however it is preferred that respondents do not provide proprietary or otherwise restricted responses.
