- Service Contract Wage Rates Attachment 2 - Scoping Vicinity Map Attachment 1-Full Curl CE Proposal Map Solicitation package Form SF-1449 (2 pages) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; a quote is being requested. Solicitation number AG-82AK-S-16-0013 is being issued as a request for quote. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-83. This project is set aside for small business concerns under NAICS code 115310 Support Activities for Forestry, small business size standard $7.5M. REFER TO ATTACHMENTS FOR COMPLETE SOLICITATION PACKAGE, MAPS AND WAGE DETERMINATION Section B-SCHEDULE OF ITEMS Item No. 1 Full Curl Hydromowing Project 80 acres* *Depending on budget, the minimum to be awarded is approximately 60 acres, the maximum is 80 acres. Section C-DESCRIPTION/STATEMENT OF WORK C.1 GENERAL C.1.1 Statement of work and specifications: The purpose of this contract is to mechanically reduce the density of small diameter trees in the 1" to 8" size class in the Pool Table area on the Rio Grande NF, Divide RD. Dominate tree species consist of Douglas fir with several other species present including aspen, spruce and white fir. Targeted species for treatment are spruce, Douglas-fir and white fir. Trees to be treated will consist of a mixture of sizes (1" to 8") in order to leave a variety of age classes remaining on site in a mosaic pattern. Stubble height (stump height) of the treated vegetation will be less than 6". Vegetation will be reduced to a mulch treatment with a minimum of 80% of the woody material (slash) less than 2" (inches) in diameter and 6" (inches) long. The mulch will be scattered across the surface in various depths but 95% of the mulch depth will not exceed 6". All work will be accomplished with a machine mounted, vertical-shaft brush cutting, thinning and shredding machine containing large (flotation type) lugged rubber tires to reduce impact to the ground, and ride over the remaining stumps and mulched vegetation. All costs to contractor (such as transportation of equipment and travel by the operator) are to be included in the cost per acre. The Contracting Officers Representative (COR) will discuss the areas to be treated and/or avoided, with the Contractor. The boundary of the project area is flagged with ribbon. The perimeter of areas of special concern such as archeological sites, will be flagged and marked as no entry areas. Treated areas may be in the form of small clear cuts, strips, mosaics, or general thinning. C.1.2 Location of Project Area - The project is within Mineral County on National Forest land and is accessed by Forest Road 600 (Pool Table Road). Legal description is T41N R1E Section 25. Size of the project area is approximately 115 acres with 60-80 acres within the project area planned for treatment. The 60-80 acres are within the boundary of the project area as shown on the vicinity and location maps attached to this contract. The boundary of the project area is marked on the ground with flagging. C.2 -DEFINITIONS None C.3 - GOVERNMENT FURNISHED PROPERTY None C.4 - CONTRACTOR PROVIDED ITEMS Machine mounted, vertical-shaft brush cutting, thinning and shredding machine containing large (flotation type) lugged rubber tires and operator. A hydro-axe or hydro-mower will meet these criteria. All transportation costs. C.5 - SPECIFIC TASKS 5.1 - A minimum of 80% of the woody material less than 2" (inches) in diameter and 6" (inches) long ("chip size" in Quality Assurance Plan). 5.2 - The mulch shall be scattered in various depths across the surface but 95% of the mulch depth will not exceed 6". 5.3 - Stubble height (stump height) of the treated vegetation will be not more than 6". C-6 - PERFORMANCE REQUIREMENTS SUMMARY SEE SECTION 6 IN SOLICITATION PKG FOR THIS INFORMATION. C.7 RESOURCE PROTECTION STANDARDS C.7.1 Water Quality - The following measures shall be observed to protect stream courses: 1. Wheeled or track-laying equipment shall not be operated in stream corridors or riparian areas except at crossing designated by the Forest Service Representative. Contractor should cross perpendicular to direction of flow. 2. Care will be taken to avoid buildup of large debris in an existing incised ephemeral stream channel below the road. The Contractor will work with the Contracting Officers Representative to avoid depositing slash into the channel by directing hydro-axe debris away from the channel during project implementation. C.7.2 Wet Period Restrictions Forest Service "all-weather" roads with a gravel or paved surface can be operated on during most weather conditions except extremely wet periods. All other roads are native surface. Travel on native surface roads and off-road with equipment is restricted when the soil is saturated. If the soils are saturated such that ruts 4 inches or deeper and 10 feet long are created by heavy vehicles, the contractor shall cease operations in that area. If conditions persist or worsen, the COR may suspend the Contract until the soils dry out or freeze. If the Contract is suspended due to wet conditions, the Government is not responsible for additional mobilization costs. C.7.3 Other Resource Protection All of the activities associated with this project shall be conducted in such a manner that there shall not be any adverse impact to the following resources listed. 1) Protection of Improvement and Survey Monuments - The Contractor(s) shall avoid any damage to improvements such as, but not limited to, fences, gates, utility poles, survey markers or monuments, survey witness trees and cultural sites. The Contractor will be required to repair or pay fair market value to replace and damaged improvements. 2) Cultural Sites - The Project Area will be surveyed for all cultural sites prior to thinning operations, however should a heritage or cultural sites be discovered during operations, the Contractors should immediately stop treatment in that area and contact the COR who will notify the Forest Archeologist. Protected cultural areas will be flagged. 3) Wildlife - If an active raptor nest is located during operations, mowing will cease and a wildlife biologist will be consulted. C.7.4 Safety When the Contractor's operations are in progress adjacent to or on Forest Service controlled roads and trails open to public travel, the contractor shall furnish, install and maintain all temporary traffic controls that provide users with adequate warning of hazardous or potentially hazardous conditions associated with the Contractor's operations. Warning signs will be posted at the junction of Highway 149 and the 600 road and also approximately 1 mile above the project along the 600 road warning Forest Users of the project work in progress. Additional warning signs must be posted at least 300 feet away from the machinery on roads and trails adjacent to the Project Area while the shredding/mowing machinery is in operation. C.7.5 Noxious Weeds Cleaning of Equipment - The Contractor shall insure all equipment moved onto Public Land is free of soil, seeds, and vegetative matter or other debris that could contain or hold seeds. The Contractor shall employ whatever cleaning methods are necessary to insure compliance and shall notify the COR prior to moving each piece of equipment onto Public Land. Notification will include identifying the location of the equipment's most recent operations. Arrangements will be made for the Forest Service Representative to inspect each piece of equipment prior to it being moved onto Public Land. "Equipment" includes all machinery except for log trucks, chip vans, pickup trucks, cars, or other vehicles used primarily for transport of personnel. C.8 PERFORMANCE TIME: The period of performance is approximately August 1 - September 1, 2016. C.9 CONTRACTOR REPRESENTATIVE: The contractor shall provide a contract representative for the performance of the work, if not themselves. The name of this person and an alternate who shall act for the contractor when the contractor is absent shall be designated in writing to the contracting officer. The contractor representative or alternate shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract. The contractor representative and alternate must be able to read, write, speak, and understand English. (OCT 2015) (a) North American Industry Classification System (NAICS) code and small business size standard. The NAICS code and small business size standard for this acquisition appear in Block 10 of the solicitation cover sheet (SF 1449). However, the small business size standard for a concern which submits an offer in its own name, but which proposes to furnish an item which it did not itself manufacture, is 500 employees. (b) Submission of offers. Submit signed and dated offers to the office specified in this solicitation at or before the exact time specified in this solicitation. Offers may be submitted on the SF 1449, letterhead stationery, or as otherwise specified in the solicitation. As a minimum, offers must show- (1) The solicitation number; (2) The time specified in the solicitation for receipt of offers; (3) The name, address, and telephone number of the offeror; (4) A technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. The offeror agrees to hold the prices in its offer firm for 30 calendar days from the date specified for receipt of offers, unless another time period is specified in an addendum to the solicitation. (d) Product samples. When required by the solicitation, product samples shall be submitted at or prior to the time specified for receipt of offers. Unless otherwise specified in this solicitation, these samples shall be submitted at no expense to the Government, and returned at the sender's request and expense, unless they are destroyed during pre-award testing. (e) Multiple offers. Offerors are encouraged to submit multiple offers presenting alternative terms and conditions or commercial items for satisfying the requirements of this solicitation. Each offer submitted will be evaluated separately. (f) Late submissions, modifications, revisions, and withdrawals of offers. (1) Offerors are responsible for submitting offers, and any modifications, revisions, or withdrawals, so as to reach the Government office designated in the solicitation by the time specified in the solicitation. If no time is specified in the solicitation, the time for receipt is 4:30 p.m., local time, for the designated Government office on the date that offers or revisions are due. (2)(i) Any offer, modification, revision, or withdrawal of an offer received at the Government office designated in the solicitation after the exact time specified for receipt of offers is "late" and will not be considered unless it is received before award is made, the Contracting Officer determines that accepting the late offer would not unduly delay the acquisition; and- (A) If it was transmitted through an electronic commerce method authorized by the solicitation, it was received at the initial point of entry to the Government infrastructure not later than 5:00 p.m. one working day prior to the date specified for receipt of offers; or (B) There is acceptable evidence to establish that it was received at the Government installation designated for receipt of offers and was under the Government's control prior to the time set for receipt of offers; or (C) If this solicitation is a request for proposals, it was the only proposal received. (ii) However, a late modification of an otherwise successful offer, that makes its terms more favorable to the Government, will be considered at any time it is received and may be accepted. (3) Acceptable evidence to establish the time of receipt at the Government installation includes the time/date stamp of that installation on the offer wrapper, other documentary evidence of receipt maintained by the installation, or oral testimony or statements of Government personnel. If the Offeror does not become registered in the SAM database in the time prescribed by the Contracting Officer, the Contracting Officer will proceed to award to the next otherwise successful registered Offeror. Section M-EVALUATION 52.212-2 Evaluation-Commercial Items (OCT 2014) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Provide narrative addressing the following evaluation criteria: 1. Technical Capability: a. Treatment Plan and Schedule - The plan and schedule should reveal how and when service items will be accomplished and why they plan to accomplish service items in that manner. Offeror is required to furnish with their proposal a Quality Control Plan. In performance based contracts, quality control is Contractor's responsibility. Contractor shall provide and maintain a complete Quality Control Plan to ensure the requirements of contract are provided as specified. The plan should identify who will be conducting the inspection, the method of inspection, the frequency of inspections and how deficiencies in work would be corrected. Once accepted, the Quality Control Plan will be incorporated into Contract. b. Experience - Offeror will be evaluated by relevant experience on the basis of its extent and depth. Offeror shall provide a detailed summary of experience with work similar to the type of work required under this project, as well as profiles and experience of company key personnel. Identify all sub-contractors, if any, their experience and what role they will be fulfilling. c. Equipment - Provide list of equipment to be used on this project. Include make/model. 2. Past Performance - Furnish a list of three (3) projects of similar magnitude completed by the firm within the last five (5) years. The list should include work similar in nature and complexity to that required by this solicitation. Include contracts with the Federal Government, agencies of State, Local Governments, and commercial customers. For each project cited, provide the following: 1. Name and location of the project; 2. Project Point of Contact's name, telephone number, email address, and fax numbers; 3. Total cost of the project; and 4. A brief narrative of the project (i.e. purpose, stream miles, terrain). The Government may or may not contact any or all references listed. For any projects listed that may report less than positive performance, you are encouraged to provide a description of the problems and efforts made to correct and prevent future occurrences. Note: Firms lacking past performance will not be rated favorably or unfavorably in this area, but will receive a neutral rating. 3. Price Technical and past performance, when combined, are equal when compared to price. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
