- Attachment 1 - Schedule of Items, Specifications and Questionnaire (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. (ii) The solicitation number for this requirement is AG-82A7-S-16-0081 and is issued as a request for quotation (RFQ) under the authority of FAR 13. (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-88 - Effective June 15, 2016. (iv) This acquisition is subject to a small business set-aside. The associated NAICS code is 333249 and small business size standard for this NAICS code is 500. (v) The Schedule of Items applicable to this acquisition provides a list of contract line item numbers, items, quantities, and units of measure. The pricing portions of this Schedule of Items should be filled out and submitted as part of any quote submitted in response to this combined synopsis/solicitation. See Attachment 1 for the Schedule of Items applicable to this acquisition. (vi) This requirement is for three (3) brand name or equal Baileigh Industrial CNC Router Tables with integrated table vacuum system, model number WR-48V, as required by FAR 11.104 (b). The three (3) CNC Router Tables with integrated table vacuum system shall be delivered FOB Destination and setup at three (3) different Forest Service Job Corps Centers. The requirement is for new equipment only, and any quote proposing used or refurbished equipment shall not be considered. See Attachment 1 for a detailed description of minimum specifications. (vii) The three (3) CNC Router Tables, and all related accessories and items, will be required (one (1) each) to be delivered FOB destination and setup by contractor once delivered. Once equipment is operational, the contractor must provide basic training to Center staff, which should include at minimum the safe operation and general maintenance of equipment. One (1) CNC Table Router with integrated vacuum table shall be provided to the following locations: Collbran Job Corps Center 57608 Highway 330 Collbran, CO 81624 Curlew Job Corps Center 3 Campus Street Curlew, WA 99118 Flatwoods Job Corps Center 2803 Dungannon Road Coeburn, VA 24230 (viii) The provision at 52.212-1, Instructions to Offerors-Commercial Items (Oct 2015), applies to this acquisition. The following addendum applies to the provision: See Solicitation Attachment 1 for additional information and required quotation submittals. 52.211-6 - Brand Name or Equal (AUG 1999) (ix) The provision at 52.212-2, Evaluation-Commercial Items (Oct 2014), applies to this acquisition. The following is the specific evaluation criteria to be included in paragraph a: 1. Price 2. Technical Capability: The contractor shall provide the technical specification of proposed equipment only if the proposed equipment are different from Baileigh Industrial, Router Table WR-48V stated above and in the specifications, and shall include it with the submission of their quote. The technical specifications will be used by the Government for comparison if different Router Tables are propsed, in order to determine the best value to the Government, price included. All contractors shall provide warranty information on proposed equipment. 3. Past Performance: This criteria will include, but may not be limited to, the evaluation of: past performance information provided by each quoter, quoter's professional references, and any other past performance information available to the Contracting Officer. (x) Quoters must electronically complete FAR provision 52.212-3, Offeror Representations and Certifications-Commercial Items (APR 2016), at www.SAM.gov before submitting the quote. Quoter must confirm in the Quoter Questionnaire (see Solicitation Attachment 1) that quoter has electronically completed the provision FAR 52.212-3, the annual representations and certifications, at www.SAM.gov. (xi) The clause at 52.212-4, Contract Terms and Conditions-Commercial Items (MAY 2015), applies to this acquisition.
