- DD Form 254 Family of Medium Tactical Vehicles (FMTV) Contractor Instructions for Requesting Classified Information The Family of Medium Tactical Vehicles (FMTV), Performance Specification contains a classified annex, Annex B "Force Protection", which is NOT available via the Internet. The classified annex will be sent via a Secure Internet Protocol (SIPR) E-Mail or registered mail; the below procedures must be followed and all requirements must be met in order to obtain a copy of the classified annex: All Offerors (or any subcontractor who will handle or have access to classified information) must have a valid United States (US) Facility Clearance (FCL) and Safeguarding Level of SECRET or higher in order to receive and temporarily store this classified annex. All classified material must be handled in accordance with the National Industrial Security Program Operating Manual (NISPOM (DoD 5220.22-M), the DD Form 254 Contract Security Classification Specification, and the PEO CS&CSS Armoring Systems Security Classification Guide (SCG), dated 31 August 2012. The posted DD254 applies to all CLASSIFIED information. Procedures for FOUO : The SCG is marked Distribution C and For Official Use Only (FOUO) and is posted via secure link for interested parties. Parties interested in obtaining the SCG must be certified in the Joint Certification Program (JCP), have an active System for Award Management (SAM) account, and submit a signed Non-Disclosure Agreement (NDA) via email to the FMTV A2 mailbox usarmy.detroit.acc.mbx.fmtv-a2@mail.mil. The data custodian in JCP must sign and submit the "FMTV Tech Data NDA" which is attached to this announcement. In addition, the interested party shall provide the Organization's name and CAGE code. Upon submission of the NDA, interested parties can request access to the SCG document by clicking on the SCG link. Upon approval, the Government will provide online access to the posted SCG as a secure file via a secure link. However, if you have already gained access to the FMTV A2 DRAFT Performance Specification by following the above instructions, you have immediate access to the SCG by clicking the appropriate link. Procedure for Classified Annex : Requests for the classified portion, Annex B "Force Protection", of the FMTV Performance Specification must be made by the offeror's Facility Security Officer (FSO) via e-mail to usarmy.detroit.acc.mbx.fmtv-a2@mail.mil with the title "Request for FMTV Performance Specification, Annex B, "Force Protection"". Requests made by anyone other than the FSO will not be processed. Firms that do not submit a proposal or who withdraw their proposals must destroy or return classified material generated or received under this Request for Proposal not later than 180 days after the opening date of proposals. Firms who submit a proposal that is not accepted by the Government or does not result in a contract award to the firm have 180 days after notification to destroy or return classified information. The following information must be provided and must match the offeror's information in the Industrial Security Facilities Database (ISFD): - The Company Name and Classified Mailing Address (must match ISFD information) - Commercial and Government Entity (CAGE) Code - Name of FSO with Telephone Number(s), E-Mail Address, and Secure SIPR E-Mail Address (if applicable) - CAGE Code(s) and Address(es) of the Company Facility (Facilities) intended to participate in performance of the FMTV Program where classified material will be utilized.
