- Project Area Map OR_WD 77-0079_Rev_51 Crushed Aggregate Specifications This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The resultant award will be a Firm Fixed-Price Purchase Order. Solicitation Number AG-04R4-S-16-0034 is being issued by the Willamette National Forest as a Request for Quotation (RFQ). This solicitation is a 100% small business set-aside with an associated NAICS code of 212319 and small business size standard of 500 employees. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-88. ITEM 1 2,525 tons of "Grading A" Crushed Aggregate Rock to be delivered to Forest Service Roads 5821, 5824-124 and 5826 on the Middle Fork Ranger District. Unit Price Note: prospective awardees MUST be registered in the System for Award Management (SAM) to be awarded a Federal Government Contract. If you are not registered, please register at the following site: https://www.sam.gov/portal/public/SAM/_______________ X 2,525 TONS = _______________ Contractor shall furnish all labor, supervision, transportation, operating supplies, and incidentals necessary to perform this project. SPECIFICATIONS: The specifications for this project are attached to the website for this combined synopsis/solicitation. Prospective Contractors are urged to review the specifications prior to submitting a quotation. DELIVERY DATES: Delivery of the crushed aggregate shall occur between July 11, 2016 and July 25, 2016. RESPONSE LOCATION: Quotations for this solicitation shall be submitted to fax number 855-890-7729 or mailed to Jeanne Williams, 2398 Kinoole Street, Hilo, HI 96720. All other inquiries and correspondence shall be submitted in writing to the Contracting Officer. (3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law. (e) (1) Notwithstanding the requirements of the clauses in paragraphs (a), (b), (c), and (d) of this clause, the Contractor is not required to flow down any FAR clause, other than those in this paragraph (e)(1) in a subcontract for commercial items.
