Description
The Network Contracting Office (NCO) 12, Great Lakes Acquisition Center (GLAC), has a requirement for Mobile Positron Emission Tomography (PET) Scan and Computed Tomography (CT) Scan Services at the Clement J. Zablocki VA Medical Center (VAMC) in Milwaukee, WI. The purpose of this solicitation is to contract for Mobile PET/CT Scan Services to be brought on-site two days per week at the Clement J. Zablocki VA Medical Center. Services shall include mobile unit with qualified vehicle operator to transport and deliver with its PET/CT Scan equipment and, as applicable, qualified technologist(s) properly certified to handle and utilize nuclear medicine equipment and perform PET/CT scans as specified in the Statement of Work (SOW). Services shall consist of all management, labor (including motor vehicle operator and qualified technologists), material, equipment (including mobile trailer/unit), supplies, medicines/pharmaceuticals, and tools necessary to perform PET/CT by contractor's specialized personnel meeting all qualifications contained in the statement of work (SOW). ALL personnel proposed for performance under this contract must be approved in advance, having met the qualification AND substantiating documentation requirements specified in the solicitation. Contractor employees accepted for performance under this program will require fingerprinting and successful adjudication of background investigation. Clinical personnel will require successful VetPro credentialing conducted by the Government. This procurement is set-aside 100% for Service Disabled Veteran-Owned Small Businesses (SDVOSB). Offerors must be verified by Vendor Information Pages (VIP) at the time of offer in accordance with VAAR 819.7003(b). The contract ordering period will be five (5) years from the date of award (anticipated October 1, 2016). All qualified offerors/firms that believe they can provide the required services as specified in the solicitation are encouraged to submit a quote. The small business size standard for North American Industry Classification System (NAICS) 621512 is $15 M. Offerors are reminded to complete registration in the System for Award Management (SAM) system, including Offeror Representations and Certifications-Commercial Items. The electronic version of this solicitation will be available on or about Tuesday, June 21, 2016. Date for receipt of quotes is Thursday, July 21, 2016 at 1:00 PM Central Time (CT). The electronic solicitation may be obtained FREE at the Federal Business Opportunities website: www.fedbizopps.gov. Any amendments to this solicitation will also be available at this address. Offerors are advised that they are responsible for obtaining any amendments which are to be submitted with their response.
