- This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation; quotes are being requested and a written Solicitation will not be issued. Solicitation number VA248-16-Q-1234 is issued as a (Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-88-1/06-15-2016. Only emailed requests received directly from the requester are acceptable. Responses should be sent to christopher.brailer@va.gov. No telephone responses will be accepted. To receive a government award, the offeror must be currently registered in System for Award Management (SAM) at www.sam.gov. Offerors are encouraged to go to the SAM database www.sam.gov and start the registration process as soon as possible to avoid possible delays should the contractor be selected as a prospective awardee. Online Representation and Certification Application (ORCA) must also be current to qualify to receive a contract award. Offeror may register with CCR at www.orca.bpn.gov NAICS and Set-aside: The NAICS is 541350 and the small business size standard is $7.5 million. This procurement is set aside for Veteran Owned Small Businesses. Any firm that is not considered a small business under this NAICS code should not submit a response to this notice. To receive consideration as SDVOSB or VOSB, the firm must be registered and verified in the VIP database at www.vip.vetbiz.gov. Contract Type: The government anticipates awarding a Firm Fixed Price award. Award will be made to lowest price quote which conforms to the requirements within this solicitation and represents the best value to the government. The Service Contact Act, Wage Determination number 2005-2113, Rev. 18, Date 12/29/2015 is applicable to this contract. For further details, access the relevant wage determination at www.wdol.gov. Description of Services: The VHA NCO 08 - Malcom Randall VAMC, ENT clinic, Gainesville, FL requires the following services: INTRODUCTION- Work covered under this contract consists in providing Commissioning Services to the project 573-16-600- Complete Campus Fire Sprinkler System, at the VA Medical Center, Gainesville Florida. STATEMENT OF WORK (see attached drawings and SPECS) General notes and expectations: a. Coordinate and direct the commissioning activities in a logical, sequential and efficient manner using consistent protocols and forms, centralized documentation, clear and regular communications and consultations with all necessary parties, frequently updated timelines and schedules and technical expertise. b. Request and review additional information required to perform commissioning tasks, including Operations & Maintenance (O&M) materials, contractor start-up and checkout procedures. Before start-up, gather and review the current control sequences and interlocks and work with contractors and design engineers until sufficient clarity has been obtained, in writing, to be able to write detailed testing procedures. c. Review submittals applicable to systems being commissioned for compliance with commissioning needs, concurrent with the design team and Construction Manager reviews. d. Review coordination drawings to ensure that trades are making a reasonable effort to coordinate. e. Write and distribute construction checklists for commissioned equipment. f. Develop an enhanced start-up and initial systems checkout plan with contractors for selected equipment. g. Perform site visits, as necessary, to observe component and system installations. Attend selected planning and job-site meetings to obtain information on construction progress. Review construction meeting minutes for revisions/ substitutions relating to the commissioning process. Assist in resolving any discrepancies. h. Witness any testing to be confident that proper procedures were followed. Include documentation in the Commissioning Record. i. Document construction checklist completion by reviewing completed construction checklists and by selected site observation. j. Document systems start-up by reviewing start-up reports and by selected site observation. k. Coordinate functional testing for all commissioned systems and assemblies. Witness and document manual functional performance tests performed by the Construction Contractor for all commissioned systems and assemblies. l. Review the preparation of the O&M manuals for commissioned equipment and review equipment warranties. m. Compile a Commissioning Record, which shall include: A brief summary report that includes a list of participants and roles, brief building description, overview of commissioning and testing scope, and a general description of testing and verification methods. For each piece of commissioned equipment, the report shall contain the disposition of the commissioning provider regarding the adequacy of the equipment, documentation and training meeting the contract documents in the following areas: Equipment meeting the equipment specifications. Equipment installation. Functional performance and efficiency. Equipment documentation and Operator training. n. All outstanding non-compliance items shall be specifically listed. Recommendations for improvement to equipment or operations, future actions, commissioning process changes, etc. shall also be listed. Each noncompliance issue shall be referenced to the specific functional test, inspection, trend log, etc. where the deficiency is documented. o. Submit 3 bound text copies of the Commissioning Records with three electronic (CD-ROM) copies. Qualifications/Certifications: The contractor shall be qualified and have the required levels of professional and technical experience to perform the work. The contractor on site and all individuals working on the project site must have taken the OSHA certified 10 hour construction safety course. Safety Regulations: All applicable safety regulations shall be observed during all work of this contract. The VA safety officer shall have full authority to see that the contractor obeys all safety rules and regulations relative to the fulfillment of this contract. Provide the necessary barricades/signage where required. Scheduling and hours of work: All work in areas involved shall be scheduled with the VA COR.Work shall be scheduled for proper execution to completion of this contract. The majority of work and maintenance can be performed during normal business hours. There is the possibility that the contractor shall perform varying work after hours and / or during the weekends at the discretion of the COR to eliminate disruption to the Medical Center. The 10 Holidays observed by the Federal Government are New Year's Day, Presidents Day, Martin Luther King Day, Memorial Day, Fourth of July, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, Christmas Day, or any other day specifically declared by the President of the United States to be a national holiday. Submitting Quote: Contractor shall submit their quote on company letterhead and shall include price and quantity of each labor category proposed, price of all anticipate indirect costs, and total price; any offered discounts; name, address, and telephone number of the offeror; terms of any express warranty; completed copy of 52.212-3 Offerors Representations and Certifications- Commercial Items, and ORCA document. Interested contractors must submit a copy of any relevant certifications and licenses along with their quote. Quotes must be received July 15, 2016 by 1300 EST. All quotes must be submitted by electronic submission in reference to VA248-16-Q-1234 to christopher.brailer@va.gov; Offers received after the exact time specified in the solicitation are considered late and may be considered if the action would not unduly delay the acquisition. All questions regarding this solicitation must be submitted to the Contracting Specialist in writing (email is preferred).
