This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a competitive 8(a) requirement for a Time-and-Materials contract. Solicitation Number: W81XWH-16-R-0045. This solicitation was issued as a request for proposals (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-88. This requirement is being solicited under the 8(a) program. The North American Industry Classification System (NAICS) number is 811219 and the small business size standard is $20,500,000.00. Contract Line Item Numbers (CLIN), Items, Quantities and Unit of Measure: Base Period: -CLIN 0001 Biomedical Equipment Technician (BMET), Administrative (ADMIN), and National Maintenance Program (NMP) Support Services -CLIN 0002 Other Direct Costs (ODCs) - Travel -CLIN 0003 Contractor Manpower Reporting Option Period 1: -CLIN 1001 BMET, ADMIN, and NMP Support Services -CLIN 1002 Other Direct Costs (ODCs) - Travel -CLIN 1003 Contractor Manpower Reporting Option Period 2 -CLIN 2001 BMET, ADMIN, and NMP Support Services -CLIN 2002 Other Direct Costs (ODCs) - Travel -CLIN 2003 Contractor Manpower Reporting Option Period 3 -CLIN 3001 BMET, ADMIN, and NMP Support Services -CLIN 3002 Other Direct Costs (ODCs) - Travel -CLIN 3003 Contractor Manpower Reporting Option Period 4 -CLIN 4001 BMET, ADMIN, and NMP Support Services -CLIN 4002 Other Direct Costs (ODCs) - Travel -CLIN 4003 Contractor Manpower Reporting Description. The U.S. Army Medical Materiel Agency (USAMMA) is seeking to award a Time-and-Materials (T&M) contract on a competitive basis to support USAMMA M3D competencies with the exclusion of other non-maintenance related services. This requirement shall provide Medical Equipment Maintenance Services, Maintenance Administrative Services, and National/Enterprise Level Technical Support And Strategic Planning Services for NMP. Periods of Performance. This contract is for one (1) eleven (11) month Base Period and four (4) twelve (12) month Option Periods. The estimated periods of performance for this contract are as follows: Base Period: 16 September 2016 - 15 August 2017 Option Period 1: 16 August 2017 - 15 August 2018 Option Period 2: 16 August 2018 - 15 August 2019 Option Period 3: 16 August 2019 - 15 August 2020 Option Period 4: 16 August 2020 - 15 August 2021 The provision at 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition. The Instructions to Offerors are located in Section L of the attached solicitation. The provision at 52.212-2, Evaluation - Commercial Items, applies to this acquisition. The evaluation criteria are located in Section M of the attached solicitation. Offerors shall include a completed copy of provision 52.212-3, Offeror Representations and Certifications - Commercial Items, in their offer. The provision at 52.212-4 Alt I, Contract Terms and Conditions - Commercial Items, applies to this acquisition. The provision at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders - Commercial Items (Deviation), applies to this acquisition. The additional clauses i, ii, iii, iv, v, vi, vii, viii, xi (A), xii, xiii, xiv, xv, are applicable to the acquisition. System for Award Management (SAM) registration is required in accordance with FAR Clause 52.204-7, System for Award Management. Lack of registration in SAM will make an offeror ineligible for award. Offerors may obtain information on registration at www.sam.gov. Additional contract requirements or terms and conditions are located in the attached solicitation. Questions concerning this Request for Proposal are due no later than 9:00am Eastern Time on 05 July 2016. Questions received after this time will not be accepted or answered. Questions shall be submitted in writing to the Contract Specialist, Mr. Christopher Sult, at christopher.m.sult.civ@mail.mil by the closing date and time as stated. The e-mail must have a subject title of "W81XWH-16-R-0045: Solicitation Questions." Telephone inquiries will NOT be accepted at any time. The Government will issue a solicitation amendment containing answers to questions. Proposals are due No Later Than: 3:00pm Eastern Time on 25 July 2016. The proposal and all copies shall be either mailed or hand carried to: Christopher Sult, Contract Specialist, U.S. Army Medical Research Acquisition Activity, 820 Chandler Street, Fort Detrick, MD 21702-5014. Proposals shall be clearly marked on the outside of any container or box with the solicitation number, the Contract Specialist name, and the offeror's company name and address. The Government will not be responsible for any lost or misdirected proposals due to failure to follow these instructions.
