- REQUEST FOR INFORMATION NAVAL MEDICAL CENTER SAN DIEGO 1.0 Description 1.1 The Naval Medical Center San Diego (NMCSD) in support of the Nutrition Management Department is seeking information on how an interested contractor could provide customized dining kits. 1.2 THIS IS A REQUEST FOR INFORMATION (RFI) ONLY. This RFI is issued solely for information and planning purposes - it does not constitute a Request for Proposal (RFP) or a promise to issue an RFP in the future. This request for information does not commit the Government to contract for any supply or service whatsoever. Further, the Navy is not at this time seeking proposals and will not accept unsolicited proposals. Respondees are advised that the U.S. Government will not pay for any information or administrative costs incurred in response to this RFI; all costs associated with responding to this RFI will be solely at the interested party's expense. Not responding to this RFI does not preclude participation in any future RFP, if any is issued. If a solicitation is released, it will be synopsized on the Federal Business Opportunities (FedBizOpps) website. It is the responsibility of the potential offerors to monitor these sites for additional information pertaining to this requirement. 2.0 Background The Nutrition Management Department requires information on customized flatware and condiment packets in support of patient dietary requirements. The contents within each packet are specific for assigned patient diets and must meet all requirements. 3.0 Requested Information 3.1 Monthly Delivery Requirements: Contractor will be responsible for ensuring delivery of the diet kits on the fifteenth (15th) of every month. The hours of operation for the Nutrition management department is 7:00 am to 1:00 pm. Deliveries shall be made between the specified delivery times. 3.2 Period of Performance: Base Year: Oct 1, 2016 - Sept 30, 2017 Option Year 1: Oct 1, 2017 - Sept 30, 2018 Option Year 2: Oct 1, 2018 - Sept 30, 2019 Option Year 3: Oct 1, 2019 - Sept 30, 2020 Option Year 4: Oct 1, 2020 - Sept 30, 2021 3.3 Description of Supplies: CLIN0001: Green Diet Kits: Deliver 25 cases per month (250/case) One (1) 13" x 13" 2-ply napkin; two (2) sugar packets, one (1) salt packet and one (1) pepper packet, one (1)yellow flex straw, one (1) heavy duty plastic black 7" fork, one (1) black knife 7.5", and one (1) black spoon 6.25". Packed in clear plastic with the word "REGULARS" written in green color on it. CLIN0002: Pink Diet Kits: Deliver 4 cases per month (250/case) One (1) 13" x 13" 2-ply napkin; 2 sugar packets, 1 salt packet, one (1)yellow flex straw, one (1) heavy duty plastic black 7" fork, one (1) black knife 7.5", and one (1) black spoon 6.25". Packed in clear plastic with the word "CLEAR LIQUID/FULL LIQUID" written in pink color on it. CLIN0003: Gray Diet Kits: Deliver 10 cases per month (250/case) one (1) 13" x 13" 2-ply napkin; 2 sugar sub packets, one (1) salt substitute - Mrs. dash brand or equal, no potassium chloride, no sodium; 1 pepper packet, one (1)yellow flex straw, one (1) heavy duty plastic black 7" fork, one (1) black knife 7.5", and one (1) black spoon 6.25". Packed in clear plastic with the word "DIABETIC/CARDIAC/RENAL" written in gray color on it. CLIN0004: Yellow Diet Kits: Deliver 8 cases per month (250/case) One (1) 13" x 13" 2-ply napkin; two ( 2) sugar sub packets, one (1) salt packet and one (1) pepper packet, one (1)yellow flex straw, one (1) heavy duty plastic black 7" fork, one (1) black knife 7.5", and one (1) black spoon 6.25". Packed in clear plastic with the word "DIABETIC" written in yellow color on it. CLIN0005: Blue Diet Kits: Deliver 15 cases per month (250/case) One (1) 13" x 13" 2-ply napkin; two (2) sugar packets, one (1) salt and one (1) pepper packet, one (1)yellow flex straw, one (1) medium grade plastic black sfork. Packed in clear plastic with the word "REGULARS, FIRST FLOOR" written in blue color on it. 4.0 Responses 4.1 Interested parties are requested to respond to this RFI through e-mail with the subject line: RFI Patient Dining Kits. 4.2 Interested parties must include business size (large or small), cage code/DUNS number and if you are not the manufacturer, please provide business size and cage code/DUNS (if applicable) of the manufacturer. 5.0 Questions Questions regarding this announcement shall be submitted in writing by e-mail to the Contracting Specialist, email address: tiffanie.l.sutgrey.civ@mail.mil.
