Requests for a solicitation will not receive a response. Responses to this Sources Sought must be in writing. The purpose of this Sources Sought Announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified businesses. Responses to this notice shall include: 1.Company Name 2.Address 3.Point of Contact 4.Phone, Fax, and Email 5.DUNS number 6.Cage Code 7.Type of business, e.g. Service Disabled Veteran Owned Small Business, Veteran Owned Small Business, 8(a), HUB Zone, Women Owned Small Business, SDB 8.Must provide a capability statement that addresses the organizations qualifications and ability to perform the work described below: The Harry S Truman VA Medical Center in Columbia, MO requires a new Radiofrequency Ablation System to be used in treatment of patients with chronic pain. The system requirements are: "The ablation system should be designed for radiofrequency procedures to include, but not limited to: Radiofrequency Ablation, DREZ, Facet Denervation, Percutaneous Chordotomy, Peripheral Neuralgia, Rhizotomy and Trigeminal Neuralgia "Must have capability to handle multiple lesions simultaneously "Must have capability to perform monopolar and bipolar procedures at the same time "Must have capability to accommodate thermal and pulsed procedures at the same time "Must have capability to deliver controlled lesions "Must have capability to provide multiple pulse settings "Must have capability to provide high temperature pulsed radiofrequency "Minimum of 120 VAC operation "Must be portable "Touch screen with easy to use interface "Allow users to save procedure specific settings for multiple providers "Must have the ability to create a large lesion acceptable for genicular and sacroiliac joint regions without increased cost of capital. "Must have the ability to create a Venom lesion by deploying electrode outside of cannula to create a "V" for larger lesion surface area. "Must have the ability to use 18 and 20 gauge Venom RF needles. Response Needed by 7/7/2016 at 1200 EST.
