- J-4 Service Log May 2015-May 2016 J-3 Past Performance Questionnaire J-2 Maintenance SOP J-1 Equipment List and Pricing Matrix Solicitation This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation number, LCLSC16P0039, is issued as an unrestricted Request For Proposals (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-88/05-16-2016. This requirement is not set-aside for small businesses. The applicable NAICS code is 811212 - Computer equipment repair and maintenance services without retailing new computers. The applicable Product Service Code is Z - Maintenance, repair, and rebuilding of equipment. This solicitation includes information for the anticipated single-award purchase order. Refer to the attached document for applicable provisions and clauses, to include Section L, Instructions to Offerors, and Section M, evaluation factors for award. Library of Congress incorporates deviations from 52.212-4 and 52.212-5 that are also included in the attached document. Quotes and questions must be submitted electronically to the contract specialist, Jeff Gangi, at jgan@loc.gov no later than the date(s) specified in the solicitation.
