- PAST PERFORMANCE INFORMATION PAST PERFORMANCE REFERENCE FORM Brand Name Justification COMBINED SYNOPSIS/SOLICITATION STATEMENT OF WORK THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN FAR SUBPART 12.6, AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTES ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. This synopsis/solicitation EA-133W-16-RQ-0610 constitutes a Request for Quotation (RFQ) and incorporates provisions and clauses in effect through Federal Acquisition Circular 2005-88. The National Oceanic Atmospheric Administration (NOAA), National Weather Service, Baltimore/Washington Weather Forecast Office (WFO), Sterling, VA requires Service Maintenance for the WFO HVAC Metasys Building Automation and Control System, as outlined in attached Statement of Work. The period of performance is for one year from August 1, 2016 through July 31, 2017, with four consecutive one-year option periods. This RFQ is being solicited as 100% Set-Aside for Small Business. The associated NAICS code is 334512, Building services monitoring controls, automatic, manufacturing, with a business size standard of 500 employees. The Federal Supply Class is Z1JZ. Please see attached Statement of Work for details. Additionally, each offeror must provide the following information with the quote: 1.The Request for Quote (RFQ) number; 2.The Name, Address, Telephone Number, and DUNS Number of the Offeror; 3.Include a statement specifying the extent of agreement with all terms, conditions, and provisions included in the RFQ. Quotes that reject the terms and conditions of the RFQ may be excluded from consideration; 4.Acknowledgment of Solicitation Amendments (SF-30), if any, and; 5.Completed FAR 209.11 on page 19 of this solicitation (Check one of the blocks in Paragraph b) 6.Offeror shall complete the certifications as applicable in the clause FAR 52.222-48 for the application of the Service Contract Act (SCA). The offeror must certify that the maintenance/service it will propose to fulfill this requirement is exempt from the Service Contract Act per conditions set forth in FAR 22.1003-4(c). If this positive certification is not provided the FAR Clauses at 52.222-41 and 52.222-42 shall be incorporated and the applicable SCA wage determination at the place of performance shall become part of any resultant contract. 7.Address technical capability in meeting the requirement outlined in the Statement of Work. 8.Provide past performance references for no more than three (3) contracts similar in size and scope. All responsible sources may submit a quotation which shall be considered by the agency. Quotes must be submitted via electronic means (e-mail) by 11:00 A.M. Eastern on July 15, 2016. Quotes must be submitted electronically via e-mail to Natasha.Thieman@noaa.gov. All questions must be submitted in writing to Natasha.Thieman@noaa.gov by 11:00 A.M. Eastern on July 12, 2016. Telephonic requests will not be honored. The anticipated award date is on or about July 20, 2016. Contractors are encouraged to register with the FedBizOpps Vendor Notification Service as well as the Interested Vendors List for this acquisition (see applicable tab within FedBizOpps where this synopsis/solicitation is posted).
