Statement of Work Bariatric/Hi-Low Beds Description of Agreement: The contractor shall provide daily rental of the Bariatric and Hi Lo Beds with the salient characteristics listed below to allow flexibility in providing comfortable, safe beds for our patients and assist in the prevention and reduction of patient injury as a result if a patient falls out of bed at the Erie, PA VA Medical Center located at 135 E. 38th St, Erie, Pennsylvania 16504: Salient Characteristics: "Height of Mattress Deck to Floor (Low Position) maximum of 10" "Height of Mattress Deck to Floor (High Position) standard at 35 ¼" "Mattress length minimum of 80" "Expandable bed frame from 36" to 42" to 48" "Mattress width expandable from 36" to 42" to 48" "Weight capacity of 1000 lbs. "Fully electric, 6-DC motor "Battery Back-up "Patient Lock Outs "Central Lock-and- Steer System with 6" swivel casters "Footboard Staff Control Panel "Auto contour function, one button chair position "Trendelenburg/Reverse Trendelenburg functions "Meets all FDA Entrapment Guidelines "Antibacterial Surfaces "Manual emergency CPR release "Tuckable quad-rails with embedded patient controls in Rails "Integrated Nurse Call (Must be able to connect to Dukane PROCARE 6000 Nurse Call System. Vendor to provide interconnect cable) "Backlit Staff Controls "Advanced power supply status indicator "Level-All Bed Reset "Integrated scale system "Integrated 3-zone Bed Exit Alarm "Nightlight "Trapeze adapter to allow compatibility with Hill-Rom trapeze "Traction and IV receptacles; drainage hooks, restraint loops PRICING: Daily rental prices shall be quoted for each type of bed. The purchase price for each bed must be provided. A pricing "cap" for future payments shall apply which automatically goes into effect when a patient's use of a product exceeds the established term. No further payments are applicable for that specific use beyond the "cap" period. The "cap" is defined and understood to be that point which the total amount of rental fees realized on a specific product for an individual patient equal the purchase price equivalent that is stated on the price/cost schedule. At which point the bed becomes the property of the VA. DELIVERY, SET UP AND PICK UP: Contractor must be able to deliver seven days per week, 24 hours per day. Contractor shall deliver and set up the specialty mattress for the appropriate patient within four (4) hours of the rental order being placed by the Erie VAMC. Contractor must pick up mattresses that are discontinued for an individual patient within 8 hours of being contacted by the Erie VAMC. FIELD SERVICE PROGRAM: Service will be provided by the Contractor's distribution center closest to the Erie VAMC. Inpatient delivery response time is four (4) hours or less from receipt of order, with an estimated delivery time given at the time of order. Inpatient pick up response time is eight (8) hours or less from receipt of order, with an estimated time given at the time of order. Barring any simple resolution or future corrective action plan, a physical on-site service response will occur in approximately 4 hours from the original service call. ROUTINE QUALITY ASSURANCE SUPPORT: Contractor staff will provide weekly quality assurance rounds to the Erie VAMC in concert with ongoing activity and on a scheduled basis. Contractor service professionals will check equipment and ensure that the equipment is operating correctly and as needed. Any and all findings will be reported in writing to the Nursing Supervisor on duty for that unit upon completion of rounds. MAINTENANCE AND REPAIR: Contractor will be responsible for providing the Erie VAMC with functional and safe equipment at ALL TIMES, and therefore will provide all routine maintenance and repair work performed on rental units at no charge to the Erie VAMC. Contractor will replace any unit that cannot be repaired with a functional unit. Response time is four (4) hours or less from receipt of maintenance order, with an estimated repair time given at the time of order. EXTENT OF OBLIGATION: The Government is obligated only to the extent of authorized rentals placed against this agreement and shall not be liable in any manner for any unauthorized purchases made by Government employees. The Contractor shall be responsible for confirming that bed rental orders are placed by authorized individuals. NOTICE OF INDIVIDUALS AUTHORIZED TO PLACE ORDERS: A list of individuals authorized to place bed rental orders under this agreement - Identified by name, work area, phone number and email - will be furnished to the Contractor by the Contracting Officer or Contracting Officer's Representative by a separate letter and updated on an as needed basis. Bed rental orders received by the Contractor from personnel not listed will not be honored. INVOICES: All invoices under this agreement will be submitted by the fifth day of the month for all rentals made during the previous month. Invoices shall contain the following minimum information: A. Name of employee who placed each order B. Patient name C. Patient room number D. Type of bed provided E. Purchase order and/or serial number F. Delivery date G. Pick Up date H. Total number of rental days I. Cost per daily rental J. Any applicable discount K. Total balance due by patient TRAINING Once the beds have been received at the Erie VAMC, the vendor will coordinate with the Contracting Officers Representative (COR), a date and time to provide an onsite in-service for the Biomedical and available Nursing staff on the bed operations to include any optional equipment. This in-service should occur within 1 week of receipt of the beds. Prior to the onsite in-service, the vendor will ensure that their employees providing the onsite in-service under this contract complete the Privacy Awareness training and submit the completion certificate(s) to the Contracting Officer Representative (COR) at the Erie VAMC. The Department of Veterans Affairs, VA must comply with all applicable privacy and confidentiality statutes and regulations. One of the requirements in VA is to have all personnel trained annually on privacy requirements. "Privacy" represents what must be protected by VA in the collection, use, and disclosure of personal information whether the medium is electronic, paper or verbal. Internet training is a requirement of the award of this contract.
