- Title: Intraoperative Monitoring System This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is HHS-NIH-NIDA-SSSA-CSS-2016-482 and the solicitation is issued as a request for proposal (RFP). This acquisition is for a commercial item or service and is conducted under the authority of the Federal Acquisition Regulation (FAR) Part 13-Simplified Acquisition Procedures; FAR Subpart 13.5-Test Program for Certain Commercial Items; and FAR Part 12-Acquisition of Commercial Items, and is not expected to exceed the simplified acquisition threshold. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005- 88, dated June 15, 2016. This acquisition is unrestricted. The associated NAICS code is 334516 and the small business size standard is 500. Background The National Institutes of Health (NIH) is the nation's leading medical research agency and the primary Federal agency whose mission is to reduce the burden of disease which they accomplish by seeking fundamental knowledge about the nature and behavior of living systems and the application of that knowledge to enhance health, lengthen life, and reduce illness and disability, conducting, supporting and making medical discoveries that improve people's health and save lives. The National Institute of Neurological Disorders and Stroke (NINDS) is a part of the NIH, and conducts research into the causes, treatment, and prevention of neurological disorders and stroke. The NINDS mission is to seek fundamental knowledge about the brain and nervous system and to use that knowledge to reduce the burden of neurological disease. The NINDS Electromyography (EMG) Section provides support services to a number of research programs including the Surgical Neurology Branch (SNB). For this project, the EMG Section provides intraoperative monitoring for complex neurosurgical cases performed by the neurosurgeons in the SNB. This service can be optimized by having a dedicated Intraoperative Monitoring (IOM) system for use in the operating room. Objective The purpose of this acquisition is to provide the National Institute of Neurological Disorders and Stroke (NINDS) Electromyography (EMG) Section with a dedicated Intraoperative Monitoring system for intraoperative monitoring of neurosurgical procedures. 1.Project Requirements: Equipment Specifications The NINDS EMG section requires one (1) intraoperative monitoring system that meets the following requirements: •Built in TcMEP stimulator (not external box), with 4 programmable outputs. The capability is for 1000V / 1500mA and shows energy transferred to patient. •IOM capabilities for SEP, MEP, tcMEP, BAEP, EMG, and EEG modalities. •A true 32 channel system (not two 16 channel systems together) which allows the ability to reference any input to any other input on the amplifier. •Sixteen high level, 2 low level programmable electrical stimulation outputs. •Noise Avoidance Program which provides the user the ability to analyze parasitic noise frequencies in the operating room and apply our algorithm to ensure the stimulus rate is such that is minimizes the effects of the aforementioned noise frequencies to physiologic responses. •Ability to run simultaneous evoked potentials at different rates including SSEP and BAEP or SSEP and triggered EMG. •Multi-task monitoring software to ensure uninterrupted monitoring throughout surgery; allowing for the ability to change settings, data views, perform measurements and calculations without interfering with vital data acquisition. •Sampling rate is 60,000 / channel •Configurable display options that allow user friendly display of several modalities concurrently •Cart-based configuration for the equipment 2.Equipment Warranty and Maintenance Specifications The NINDS EMG Section requires that the above-described intraoperative monitoring system come with the following: •One year warranty from installation completion date which provides the following support services. oUnlimited phone support with Webex capability for assistance with software support, updates, programing questions, if needed. oReplacement of all intraoperative monitoring system parts as needed during manufactures warranty (keyboards, mouse, USB not included. oProvision and installation of equipment software version upgrades as released oUnlimited e-mail and telephone customer support for scheduled and emergency services between the hours of 8:00 am and 5 pm, local prevailing time, Monday through Friday. oDedicated call prompt for immediate support while in the operating room. oOn-site service visits by an OEM certified technician/engineer if required •Please note if there is the ability to continue maintenance through separate service agreement after one year warranty has ended. 3.Training Requirements •On-site customer training in the use of the equipment is required. •Training shall be for clinical staff and technicians in the EMG and EEG section and shall be a duration of at least four hours. • Training will occur at the customer site within six (6) weeks of equipment product delivery. •Training shall take place between the hours of 8:00am and 5:00pm, Bethesda, MD local prevailing time, Monday through Friday. •On-site training must take place at the customer site within two (2) weeks of equipment installation. Training shall take place between the hours of 8:00am and 5:00pm, Bethesda, MD local prevailing time, Monday through Friday. Delivery Delivery and installation of the above detailed equipment is required within thirty (30) days after contractor receipt of order, on or about August 15, 2016. All equipment, including equipment shipped separately, must be received within the thirty day period. Delivery must be FOB Destination and must include inside installation. The equipment shall be delivered and installed between the hours of 8:00am and 5:00pm, Bethesda, MD local prevailing time, Monday through Friday. Place of Performance Delivery shall be made to Dr. Tanya Lehky, NINDS, Building 10, Room 7-5753, 10 Center Drive, MSC 1404, Bethesda, MD 20892 between the hours of 8:00 AM and 4:00 PM local time Monday - Friday. Contract Type A firm fixed price purchase order is anticipated. FAR clause 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition. FAR clause 52.212.-2, Evaluation - Commercial Items, is applicable to this requirement. (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: The government will award a purchase order resulting from this solicitation on the basis of best value, technical factors, past performance and price considered. Technical factors and past performance shall be considered more important than price. However, the government reserves the right to award to the offeror who represents the best value for the government. The technical evaluation factors are as follows, in order of importance: 1)Factor 1: Equipment Capability The contractor shall detail in its technical proposal how it shall meet each of the project requirements. This shall include detailed specifications of the offered equipment and its capabilities. Technical approach shall be evaluated for the ways in which the proposed equipment meets or exceeds the equipment specification requirements. Exceeding requirements shall be considered favorably, as shall ability to upgrade the system to super-resolution capabilities in the future if desired. 2)Factor 2: Equipment Warranty and Extended Warranty Coverage The contractor shall detail in its technical proposal the terms and conditions for its proposed warranty and maintenance coverage. The proposed approach shall be evaluated for the ways in which it meets or exceeds the equipment warranty specifications. 3)Factor 3: Delivery, Installation, and Training The contractor shall detail in its technical proposal how it shall meet the delivery, installation, and training requirements. Proposals must include a delivery lead-time and detailed explanation of who shall perform installation, the installation time frame, as well as what training will be provided and how it shall be delivered. Past performance shall be demonstrated by providing reference as evidence of timely delivery, installation and training on the same or similar equipment. (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. (End of Provision) Offerors must include a completed copy of the provision at FAR clause 52.212-3, Offeror Representations and Certifications - Commercial Items, with its offer. FAR clause at 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition. FAR clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition. FAR Clause 52.237-2, Protection of Government Buildings, Equipment and Vegetation (May 2014) The following Contracting Officer's Representative (COR) will represent the Government for the purpose of this contract: TBD at time of award The COR is responsible for: (1) monitoring the Contractor's technical progress, including the surveillance and assessment of performance and recommending to the Contracting Officer changes in requirements; (2) interpreting the statement of work and any other technical performance requirements; (3) performing technical evaluation as required; (4) performing technical inspections and acceptances required by this contract; and (5) assisting in the resolution of technical problems encountered during performance. [The alternate COR is responsible for carrying out the duties of the COR only in the event that the COR can no longer perform his/her duties as assigned.] The Contracting Officer is the only person with authority to act as agent of the Government under this contract. Only the Contracting Officer has authority to: (1) direct or negotiate any changes in the statement of work; (2) modify or extend the period of performance; (3) change the delivery schedule; (4) authorize reimbursement to the Contractor for any costs incurred during the performance of this contract; (5) otherwise change any terms and conditions of this contract; or (6) sign written licensing agreements. Any signed agreement shall be incorporated by reference in Section K of the contract The Government may unilaterally change its COR designation. The Defense Priorities and Allocations System (DPAS) are not applicable to this requirement. Responses to this solicitation must include sufficient information to establish the interested parties' bona-fide capabilities of providing the product or service. The price quote shall include: unit price, list price, shipping and handling costs, delivery days after contract award, delivery terms, prompt payment discount terms, F.O.B. Point (Destination or Origin), product or catalog number(s); product description; and any other information or factors that may be considered in the award decision. Such factors may include: past performance; special features required for effective program performance; trade-in considerations; probable life of the item selected as compared with that of a comparable item; warranty considerations; maintenance availability; and environmental and energy efficiency considerations. In addition the Dun & Bradstreet Number (DUNS), the Taxpayer Identification Number (TIN), and the certification of business size must be included in the response. All offerors must have an active registration in the System for Award Management (SAM) www.sam.gov." All responses must be received by July 18, 2016 at 8:00 am EST and must reference number HHS-NIH-NIDA-SSSA-CSS-2016-482.
