Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number is VA258-16-Q-0598 and is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-88 effective 06/15/16. The associated North American Industrial Classification System (NAICS) code for this procurement is 339112 with a small business size standard of 1000 employees. This posting is notification of a sole source award to Olympus America, Inc., sole source justification attached. FOB destination: New Mexico VA Health Care System, 1501 San Pedro Dr SE, Albuquerque, NM 87108-5128. The following products needed are: one (1) Olympus BF-H190 EEIII High Def Diagnostic IC Bronchoscopes one (1) Olympus BF-P190 EEIII Slim Bronchoscope two (2) Olympus MAF-TM Airway Mobilescope-Treatment/Asp one (1) Olympus MAF-GM Airway Mobilescope-Intubation Bronchoscopes. Interested vendors are directed to block 20 of the Standard Form 1449, the first page of solicitation VA258-16-Q-0598, for specifics. Providing price quotes for all line items as listed in the Price/Cost Schedule is required. ***Question Submission: Interested offerors shall submit all questions concerning the RFQ at the earliest possible time to enable the Buyer to respond. Questions shall be submitted via email to: Bryan.Rogers2@va.gov. Amendments to the RFQ addressing inquiries or concerns will be posted as an amendment to the RFQ on this same website. Questions not received within a reasonable time prior to close of the RFQ may not be considered. *** New Product only; NO remanufactured or "gray market" items. All items shall be covered by manufacturer's warranty. Quote SHALL be valid for a minimum of 60 calendar days after due date of quotations. This solicitation requires registration by the vendor with the System for Award Management (SAM) prior to award, pursuant to applicable regulations and guidelines. Registration information can be found at www.sam.gov. Failure to comply with the terms and conditions as stated in this announcement may result in offer being determined as non-responsive. The selected Offeror shall comply with all clauses and provisions as listed in the RFQ.
