- Required Insurance, Local Instruction Wage Determination for Lowndes Country, GA Disclosure of Lobbying Activities, Standard Form LLL Required Information prior to potential contract award Performance Work Statement for MDG Medical Linens at Moody AFB Pricing Schedule for MDG Medical Linens This is a combined synopsis and solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and A WRITTEN SOLICITATION WILL NOT BE ISSUED. AVAILABILITY OF FUNDS FAR 52.232-18 --AVAILABILITY OF FUNDS Availability of Funds (Apr 1984) The Government intends to award for this service, HOWEVER FUNDS ARE NOT PRESENTLY AVAILABLE FOR THIS CONTRACT. The Government's obligation under this contract is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise until funds are made available to the Contracting Officer for this contract and until the Contractor receives notice of such availability, to be confirmed in writing by the Contracting Officer. (End of Clause) All references to offer/offeror are deemed to have the meaning of quote/quoter and any quote received in response to this RFQ is not binding until contractor signs the purchase order issued at time of award or starts performance. The solicitation number for this procurement is FA4830-17-Q-Z002 and is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-88, Defense Acquisition Circular (DAC) 91-13 (Correction) and Air Force Acquisition Circular (AFAC) 2016-0603. The North American Industry Classification System (NAICS) is 812331. The small business size standard is $32.5 Million. This acquisition will be dissolved of any small business set-aside in accordance with Federal Acquisition Regulation (FAR 19.1505). Local clauses can be viewed at Attachment 3: Moody Instructions. Description: The Contractor shall provide MDG LINEN CLEANING AND SCRUB RENTAL SERVICES for the 23 Medical Group (MDG) located at Moody AFB, GA, also referred to as the Medical Information Services Support (MISS). The MDG Linen Services shall be performed to meet standards and provide support consistent with the policies, procedures, and standards of the Department of Defense (DoD), Air Force (AF), 23d Wing, and the 23d Medical Group. See Attachment 1: Performance Work Statement (PWS) for full description of the requirement. Quotations must be submitted on Attachment 4: Pricing Schedule which indicates the period of performance for each period. FOB Point: Destination (Moody AFB, GA) Provision at FAR 52.212-1, Instruction to Offerors- Commercial items applies to this acquisition, except that paragraph (c) is omitted. A Request for Quotation (RFQ) should be submitted and shall contain the following information: 1. RFQ Number; 2. Time Specified for Receipt of Quotations; 3. Name, Address, Telephone Number of Quoter; 4. Name and Email address of authorized representative to discuss quote; 5. Any discount terms and acknowledgement of all solicitation amendments (if applicable). Quoted prices must be submitted electronically using the Pricing Schedule at Attachment 4. Quoters shall insert unit prices in the attached pricing schedule, rounded to the nearest cent (for example tiny_mce_marker.02 not tiny_mce_marker.0231). Quoters rejecting the terms and conditions may be excluded from consideration. Late Quotations: Request for quotations or modification of quotations received at the address specified for the receipt of quotations after the exact time specified for receipt of quotations may not be considered. All quoters must be System for Award Management (SAM) registered, to include the online representation and certification application. Quoters that fail to furnish the required representation information via submission or SAM registration, or reject the terms and conditions of the RFQ, may be excluded from consideration. (See Attachment #2) Response Time: Quotations will be accepted at the 23d Contracting Squadron located at 4380B Alabama Rd, B932, Moody AFB, GA 31699 no later than 3:00 PM Eastern on Wednesday, 6 August 2016. All quotations shall be marked with the RFQ number and title. Email quotations, to be sent to both points of contact, will be accepted and are encouraged; however, no facsimile quotations will be accepted. To avoid confusion, questions previously submitted must be resubmitted if they have not been addressed in solicitation amendment. Any questions from industry must be received in writing to the primary and alternate email addresses listed below no later than 4:00 PM Eastern on 20 July 2016. No official site visit will be conducted. Evaluation: IAW FAR 52.212-2, Evaluation-Commercial Items, the Government will award a contract to the quoter whose quotation is judged to represent the best value to the Government based on Price (summation of base and all option years) and Technical Factors. Quotes will be evaluated using Lowest Price Technically Acceptable (LPTA). Price: Evaluation of price will consider the total of the annual prices for all contract years (base plus all options, including a possible six-month extension), with each year's price evaluated as the sum of the prices of all contract line items (CLINs) for that year. Each CLIN price will be evaluated as the quantity multiplied by the unit price, rounded to the nearest cent. A potential six-month extension IAW the clause FAR 52.217-8 will be evaluated at the pricing of the final option year. In the event of an extension under this clause, it will be exercised at the prices of the prior contract year. Technical Factors: As part of the technical evaluation, the quoter must submit a plan that clearly demonstrates an Essential Services Plan IAW DFARS 252.237-7023 and the quoter shall submit a technical approach that includes an Essential Services Plan and an Inspection of Laundry Services Plan. a. Essential Services Plan- The Essential Services Plan must include an explanation of how the offeror will undertake steps to maintain the service in the event of a qualifying event or emergency IAW DFARS clause 252.237-7024. The plan must address; i. Challenges associated with maintaining essential contractor services during an extended event, such as a pandemic that occurs in repeated waves; ii. The time lapse associated with the initiation of the acquisition of essential personnel and resources and their actual availability on site; iii. The components, processes, and requirements for the identification, training, and preparedness of personnel who are capable of relocating to alternate facilities or performing work from home; iv. Any established alert and notification procedures for mobilizing identified "essential contractor service" personnel; and v. The approach for communicating expectations to contractor employees regarding their roles and responsibilities during a crisis. b. The Laundry Services Plan must address how the contractor plans to meet each section of Inspection of Laundry Services checklist items on page 18 of PWS. i. Section A: Plant Facility ii.
