MICC, Contracting Office, Fort Polk, LA 71459 06 Jul 2016 FSC/PSC: U006 SUBJECT: Combined Synopsis/Solicitation - SHARP Training RFQ reference number # POLK-0010844136 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quotation (RFQ). Submit written quotes on RFQ reference number # POLK-0010844136 This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-88. This solicitation is set-aside as 100% small business. The North American Industry Classification System (NAICS) code is 611430, Professional and Management Development Training, size standard 11.0 Mil DESCRIPTION This requirement is for comprehensive alcohol, drug, DUI, sexual assault and suicide prevention training that will be tailored to Fort Polk Service members, dependents and civilians. Must be able to provide a Subject Matter Expert with an understanding based on personal testimony of the dangers of high-risk drinking and substance abuse including specific strategies to prevent risk to a Service member's mission and/or career. The goal is also to reduce cases of sexual assault, DUI, alcohol-related violence and suicides. The Statement of Objectives is included below. Potential offerors are to quote on the line items that are provided below: CLIN 0001 Prevention Training Service- Alcohol, Drug, Suicide Prevention training. Training to be delivered on 24-26 Aug 2016. Three Programs per day for a total of six (6) programs. CLIN 0002 Prevention Training Service- Alcohol, Drug, suicide Prevention training. Training to be delivered on 06-08 Sep 2016. Three Programs per day for a total of (6) six programs. FAR 52.212-1, Instructions to Offerors -- Commercial Items (Oct 2015), applies to this acquisition and any addenda to the provision. FAR 52.212-2, Evaluation--Commercial Items (OCT 2014), applies to this acquisition. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: a. Technical Capability: Each vendor shall provide company information, including previous live training experience to include military venues and references. Provide a training syllabus and applicable training brochures. Vendor shall provide a description of mechanism used to capture audience feedback. The information provided should be presented in such a manner as to enable the Government to make a thorough and complete evaluation and arrive at a sound determination as to whether or not the products proposed and described will satisfy the requirements of the Government as stated in this request for quote. b. Price Submission: Vendors shall submit one original copy of their quote. The vendor's proposed price will be evaluated on the basis of price reasonableness. The Government will evaluate each vendor's Written Technical submission acceptable (pass) or unacceptable (fail). 1. Technical Acceptability - Technical acceptability will be determined based on an evaluation of the vendor's technical submitssion. Written Technical Submittal: Pass (P): Technical submission demonstrats that the information quoted meets the Government's requirements as set forth in this Request for Quote. Fail(F): Failing to provide any of the technical information and/or providing unclear or inadequate information proposed that does not meet the Government's requirements. NOTE: If the Contracting Officer determines that a vendor's Written Technical Submission is "unacceptable," that vendor's entire submittal may be determined to be technically unacceptable and may no longer be considered for further competition or award. 2. Price The vendor's proposed price will be evaluated on the basis of price reasonableness. The evaluation may include, but is not limited to: a. Price comparison with other offerors; b. Comparison with independent Government cost estimates. The Government intends to award to one vendor on an all or none basis. A written notice of award or acceptance of an offer mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer has specified expiration time, the Government may accept an offer whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. The Government intends to make award on or about 01 Aug 2016 with a contract period of 24 Aug 2016 - 30 September 2016. STATEMENT OF OBJECTIVES (SOO) PART I General Information C.1.0 Scope: The Contractor shall provide the following training: • Comprehensive alcohol, drug, DUI, sexual assault and suicide prevention training • Presentation must be tailored to Fort Polk Service members, dependents and civilians • Must provide a Subject Matter Expert with an understanding based on personal testimony of the dangers of high-risk drinking and substance abuse including specific strategies to prevent risk to a Service member's mission and/or career. The goal is also to reduce cases of sexual assault, DUI, alcohol-related violence and suicides. The training shall consist of the following components: Alcohol and Substance Abuse, Safety and DUI Prevention, Suicide Prevention, Sexual Assault and Domestic Abuse. The training shall consist of classroom instruction. The language for conducting the presentations shall be in English. C.1.1 Period of Performance: • The period of performance for the training is three (3) consecutive days consisting of two (2) trainings per day x two (2) separate iterations of training totaling 12 training sessions • This training must occur on 24-26 Aug 2016 and 06-08 September 2016. C.1.2 Place of Performance: The place of performance shall be at Fort Polk, Louisiana C.1.3 Contractor Travel Responsibilities: The Contractor shall arrange for the round-trip airfare, ground transportation, lodging and per diem for the presenter providing this training. C.1.4 Security Requirement: Instructor shall meet all applicable base security obligations for conducting training at Fort Polk. PART 2 CUSTOMER/CONTRACTOR FURNISHED PROPERTY, EQUIPMENT, SERVICES, AND DOCUMENTATION C.2.0 Customer Provided Property/Equipment/Information/Personnel: The Customer will provide the following: • One venue and appropriate audio/visual support to accommodate personnel being trained for each presentation • Required documentation required for the instructor to enter Fort Polk facilities C.2.1 Contractor Provided Property/Equipment/Information/Personnel: • The Contractor shall provide training information in English PART 3 SPECIFIC REQUIREMENTS C.3.0 Vendor Tasks • The Contractor shall conduct three (3) consecutive days of two (2) presentations each on alcohol, drug, DUI, sexual assault and suicide prevention topics • The Contractor shall complete two (2) iterations of training totaling 12 presentations. C.3.1 The Contractor must possess the following capabilities: • Must have personal experience of alcohol and substance abuse and be alcohol and drug free for a minimum of 20 consecutive years • Must have personal experience and detailed knowledge of suicide or suicidal ideations and provide tactical solutions to reduce and prevent • Must have personal experience and knowledge of the risks of driving under the influence of alcohol and provide tactical solutions to reduce and prevent • Must have personal experience of law enforcement penalties and risks affiliated with driving under the influence charges and arrest • Must have a minimum of 50 military commander and Prevention Specialist letters of recommendation • Must provide analytical audience surveys of trainings presented in 2015 documenting a 99% success rate of program's impact on Service members • Must base a minimum of 75% of training material on his/her personal experience and not those of a third party (eg: friend, relative, etc) • Must have presented a minimum of 200 speaking presentations to adults between the ages of 18-24 yrs old (high-risk target group) • Must have presented a minimum of 200 military alcohol, drug, DUI, sexual assault and suicide prevention trainings and understand military values and dialogue • Must solidify the value of this product by producing a minimum of 10 non-biased and professional news/media evaluations • Must provide a minimum of six (6) "positive" military commander/civil servant letters of referral • Must provide minimum of 1000 documented and verifiable comments or feedback from military Service members whom have attended the training program • Must have extensive experience with connecting with an audience in a way that will keep audience focused and engaged throughout the training • Must be experienced and credentialed whose expertise is in alcohol, drug, DUI, sexual assault and suicide prevention, bystander intervention, resiliency and Keeping What You Have Earned Campaign • Must provide the same presenter for all trainings throughout contract for consistency • Must NOT use PowerPoint or statistics in their presentations • Must have knowledge of resources available to those in need of alcohol or substance abuse counseling or treatment Plan of Instruction: In conjunction with submission of the quote, the Contractor shall submit a Training Outline along with their quote. Training Assessment Report: Within 10 days of course completion, the Contractor shall submit a Training Assessment Report.
