Description

This is a Solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation; proposals are being requested and a written Solicitation is included in this posting. The solicitation number is VA263-15-R-0126 and is issued as a request for proposal (RFP). This requirement is unrestricted. The associated North American Industrial Classification System (NAICS) code for this procurement is 621498, "All Other Outpatient Care Centers," with a size standard of $20.5 million. To be considered for award of a Government contract, the offeror must be currently registered in System for Award Management (SAM) at www.sam.gov. Offerors are encouraged to go to the SAM database www.sam.gov and start the registration process as soon as possible to avoid possible delays should the contractor be selected as a prospective awardee. The Department of Veterans Affairs, Network Contracting Office 23 is soliciting proposals for two Community Based Outpatient Clinics (CBOC). The resulting contract will require the contractor to provide Primary Care and telehealth support services for two Community Based Outpatient Clinics (CBOC), in accordance with the specifications, terms and conditions of the solicitation. One CBOC is to be located within the city limits of Williston, ND and the other CBOC is to be located within the city limits of Dickinson, ND. The contractor shall establish two separate locations to provide the required services specified under this contract. Veterans to be serviced under the Williston, ND contract reside primarily in the North Dakota Counties of: Williams, McKenzie, Mountrail, Divide, Burke and Dunn and the Montana Counties of Sheridan, Roosevelt and Richland but are not restricted to those counties. Veterans to be serviced under the Dickinson, ND contract reside primarily in the North Dakota Counties of: Golden Valley, Billings, Stark, Hettinger, Slope, Bowman, Adams and the South Dakota counties of Harding, and Perkins. CBOC services will be provided to beneficiaries of the Fargo VA Health Care System. Payment for services will be based on a monthly Capitated Rate. The anticipated period of performance will be from March 1, 2017 (or date of award) through February 28, 2018 for the Base Year, with four (4), one year option renewal periods. Option periods are exercised at the sole discretion of the Government. The date for receipt of proposals is no later than August 4, 2016, 3:00 p.m. Central Standard Time.
