Description

This solicitation amendment for Solicitation Number VA258-16-Q-0599 is being issued to clarify the Quote submission due date and time. Quotes are due July 12, 2016 at 12:00 p.m. EST and should be submitted by email to Maribel Ruble, Contract Specialist at Maribel.Ruble@va.gov. Other Terms and conditions of the solicitation remain unchanged. The Department of Veterans Affairs, SAO West - Network Contracting Office 18 (NCO 18) has a requirement for one (1) Service Contract for DCAD 3.1 Prostate Server Software includes maintenance, technical support, hardware and software and DCAD 3X Tower Server Hardware no Display, S/N 66848737. This service contract is needed to service and maintain an existing operational prostate cancer screening station in the radiology department at the Phoenix VA healthcare system. The services are for the Department of Veterans Affairs Phoenix VA Health Care System (PVAHCS), 650 E Indian School Road, Phoenix, AZ 85012-1839. The contract will include a base year with four (4) one-year options that can be exercised at the discretion of the Government. Base Year: August 1, 2016 through July 31, 2017 Option Year One: August 1, 2017 through July 31, 2018 Option Year Two: August 1, 2018 through July 31, 2019 Option Year Three: August 1, 2019 through July 31, 2020 Option Yea Four: August 1, 2020 through July 31, 2021 This is a Combined Synopsis/Solicitation (CSS) for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-88, Effective: 15Jun2016 and VAAR Parts Current to 2008-02 and 78 FR59865 changes date 30Sep2013. Offerors that fail to furnish required representation or technical information as required by Federal Acquisition Regulation provisions 52.212-1 or reject the terms and conditions of this solicitation may be excluded from consideration. It is the responsibility of the contractor to be familiar with the applicable provisions and clauses which can be accessed in full text at www.farsite.hill.af.mil. This solicitation is being issued under Solicitation Number VA258-16-Q-0599 which will result in a FFP Contract. This action is being solicited as a unrestricted. Quotes will be evaluated on a Lowest Price Technically Acceptable (LPTA) basis. Award will be made on an all or none basis. An offeror must provide sufficient technical information on all items requested to be eligible for award. All responding vendors must be actively registered with System for Award Management (SAM) prior to award of contract. Information to register in SAM can be found at https://www.sam.gov/portal/public/SAM. Delivery and Shipping shall be included in the price. All items and services shall be shipped FOB Destination to/or contact to the following location: PVAHCS 650 E. Indian School Road Bldg. 1, Rm. 1163A Phoenix, AZ 85012-1892. Quotes are due July 12, 2016 at 12:00 p.m. EST and should be submitted by email to Maribel Ruble, Contract Specialist at Maribel.Ruble@va.gov.
