Description

This is a Solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation; quotes are being requested and a written Solicitation is included in this posting. The solicitation number is VA263-16-Q-0637 and is issued as a request for quote (RFQ). The associated North American Industrial Classification System (NAICS) code for this procurement is 621511, "Genetic Testing Laboratories," with a size standard of $32.5 million. To be considered for award of a Government contract, the offeror must be currently registered in System for Award Management (SAM) at www.sam.gov. Offerors are encouraged to go to the SAM database www.sam.gov and start the registration process as soon as possible to avoid possible delays should the contractor be selected as a prospective awardee. The Department of Veterans Affairs, Network Contracting Office 23 is soliciting quotes for a Firm Fixed-Price (FFP) contract to an offeror capable of providing Male Fertility Testing for VA beneficiaries of VA NWI Healthcare System, Omaha, Nebraska. All work is to be performed at the offeror's site. Offeror is responsible for providing all supplies, materials, equipment and labor to perform the testing services. The anticipated period of performance will be from October 1, 2016 (or date of award) through September 30, 2017 for the Base Year, with four (4), one year option renewal periods. Option periods are exercised at the sole discretion of the Government. The date for receipt of offers in no later than August 10, 2016, 1:00 p.m. Central Standard Time.
