***** Begin Word Document - 'FBO-DESCRIPTION' ***** Hemodialysis Concentrate The Department of Veteran Affairs (VA) is proposing to issue a Multiple Award BPA for One (1) Base Year and Four (4) one (1) year option periods for Hemodialysis Concentrate. This requirement is an Open Market. The BPA, including product, prices, and other terms and conditions, is public information and is releasable under the Freedom of Information Act (FOIA). Procurement related information, including negotiations, shall be treated as confidential and shall not be disclosed except as required by applicable laws and regulations. The BPA holder shall provide written notice to the Strategic Acquisition Center (SAC) BPA administrator, of any changes to any items on this resultant BPA. Changes include, but are not limited to, product deletions, and/or change in vendor company information, including contract administrator, company name, address and phone number.
