- Federal Supply Code (FSC) 5610, North American Industry Classification System (NAICS) Code 212319, Other Crushed and Broken Stone Mining and Quarrying. Aggregate Stone Products, in accordance with the Statement of Work that will be furnished with the Request for Proposal (RFP) solicitation. This request for information is based on a known requirement for Aggregate Railroad Material. This aggregate must be Type "A" base course, No. 4 ballasts, and screening used in railroad maintenance efforts and on the of Oklahoma Department of Transportation (ODOT) approved aggregate stone products list. Specifically, the Type "A" Base Course material must comply with the most current regulation/specification set forth by the ODOT, Reference Section 703.01, Aggregate for Aggregate Base. Furthermore, the Aggregate must originate from a site that is shown on the ODOT website page, "Materials and Testing e-Guide - All Approved and Unexpired Aggregate/Stone Listings", or provide documentation from ODOT that the site is approved to supply the requested aggregate, per the ODOT specifications and testing. See http://www.ok.gov/odot/Doing_Business/Construction/Materials_&Testing_e-Guide. The end user is the McAlester Army Ammunition Plant located in McAlester, OK. FOB Destination. All vendors must provide the following information: 1. Statement of your firm's intention to bid and how it intends to meet the requirements stated above 2. DUNS, CAGE Code, Tax ID, and Point of Contact Information 3. Number of employees (for business size verification) 4. Manufacturing plant name and location (for U.S. based status verification) 5. Market Conditions 6. Terms and Conditions (include any commercial standard terms and conditions) 7. Production Capability, Economic Ordering Quantity, and Delivery Lead Times 8. Warranty 9. Minimum Procurement Quantity required for economical production 10. Delivery capacity of truckloads per week (Monday through Thursday 6 a.m. to 3 p.m.) Please take into consideration security and receiving requirements at MCAAP. 11. Hauling method (size of trucks and maximum tonnage) 12. Proximity of an ODOT approved source for MCAAP requirements 13. Availability of a specification sheet, from an ODOT approved source, to submit with your firm's proposal. 14. Ease of hiring a local contractor to perform delivery All of the above information must be submitted in detail. If a contract will be competed, it will be published electronically as a solicitation to the FBO website and will be the responsibility of the contractors to check regularly.
