- RFQ - Updated 15 July 16 PWS - Updated 15 July 16 This is a Combined Synopsis/Solicitation for a commercial service prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. SOLICITATION #: FA4861-16-Q-A010 This combined synopsis/solicitation is issued as a Request For Quotes. A Firm Fixed Priced contract will be awarded. Provision Addenda: All quotes shall include the following minimum information: Product Specifications, Cage Code, Duns Number, Taxpayer I.D. No., Payment Terms, and/or Discount Terms, Offerors Reps and Certs, Point of Contact, Phone, Fax, Email and Proposed Delivery Schedule. IAW FAR 52.204-7 and DFARS 252.204-7004 Alt A, prospective vendors must be registered in the SAM database prior to contract award. Prospective vendors should visit the SAM website at https://www.sam.gov/portal/public/SAM/ to register. The Contractor shall submit a lesson plan identifying the mandatory topics that must be covered per Title 49, Subtitle B, Chapter l, Subchapter D, Part 195, Subparts F & G. The contractor must identifying within the lesson plan when a hands on portion, practical, and knowledge based exam are being used which will be used to enhance the learning outcomes. The contractor must identify the number of instructors and shall provide documentation qualifying that contractor to instruct said such topics. The contractor must develop a plan that will not exceed 5 days of instruction. Additionally, the Contractor shall comply with all the requirements in the Performance Work Statement. Lastly, the contractors who do not quote the entire package requested will be considered non-responsive and their quotes will not be evaluated. Addendum 52.212-1: To be considered for award, the contractor must submit: 1. Completed RFQ(See attached template) 2. Lesson Plan on topics per Title 49, Subtitle B, Chapter l, Subchapter D, Part 195, Subpart F&G 2. Hands on, practical, and knowledge based exam information identified within the lesson plan. 3. Number of Instructors with instructors qualifications 4. Training schedule not to exceed 5 days. FAR 52.212-2 - Evaluation - Commercial Items: The Government will award a contract resulting from this solicitation to the responsible offeror whose offer is the lowest price, technically acceptable. Technical acceptability is based on the contractor's submission of the required documents in the addendum 52.212-1 as well as the contractors compliance with all the requirements stated within the Performance Statement of Work. This is an "all or none" requirement and will be awarded on one contract. Multiple contracts will not be issued. FAR 52.212-3 - Offeror Representations and Certifications -- Commercial Items : The offeror shall complete only paragraphs (b) of this provision if the Offeror has completed the annual representations and certification electronically via the System for Award Management (SAM) Web site accessed through http://www.acquisition.gov. If the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete only paragraphs (c) through (r) of this provision. FAR 52.212-4 - Terms and Condition - Commercial Items: Applies to this acquisition. FAR 52.212-5 - Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (July 2014): Applies to this acquisition. AFFARS 5352.201-9101 - Ombudsman (a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions). (c) If resolution cannot be made by the contracting officer, concerned parties may contact the Center/MAJCOM or AFISRA ombudsmen, [Insert names, addresses, telephone numbers, facsimile numbers, and e-mail addresses].
