EXCAVATION PERMIT SOLICITATION ATTACHMENT 5 ENGINEERING FORM 4025-R SOLICITATION ATTACHMENT 4 BID SHEET SOLICITATION ATTACHMENT 3 DRAWINGS SOLICITATION ATTACHMENT 3 WAGE RATE SOLICITATION ATTACHMENT 1 Solicitation SP4702-16-R-0022 The Defense Logistics Agency, Defense Supply Center has a requirement for Maintenance and Repair to Pavement located at the Defense Supply Center Columbus, Ohio. This procurement is issued as a 100% small business set aside. The NAICS for this requirement is 237310. The Small Business size standard for this requirement is $36,500,000.00. The award will be a firm fixed contract. Award will be based on the lowest evaluated price of the technically acceptable proposal meeting all of the solicitation requirements from responsible offerors with acceptable past performance. There will be one award made for this project. Award may be made without discussions. The following summary is for the purpose of general information and is not intended to include or describe every feature or item or to define the scope of work. It is expected that prospective contractors will attend the site visit, review solicitation documents, and thoroughly familiarize themselves with the project prior to submitting a proposal. One preproposal conference/site visit is scheduled for this project and is scheduled for 1:00 PM local time on 27 July 2016. All individuals wishing to attend will be required to request Center access no later than 24 hours prior to the site visit date. Those individuals wishing to attend must provide their name and corporate affiliation to barbara.bocsy@dla.mil by email. The Defense Supply Center is a secure facility and individuals who do not preregister will not be granted access. Registered participants can not bring unregistered personnel on center with them. Individuals will need to enter the Center through the Visitor Processing Center at 401 North Yearling Road and valid federal or state issued photo identification will be required. PROJECT INFORMATION: The Request for Proposals will solicit for a single award firm fixed priced contract for work to be performed at the Defense Supply Center Columbus located in Columbus, OH. SCOPE: This project consists of providing asphalt pavement maintenance and repair measures by utilizing Sealing of Cracks, Cold Milling and Resurfacing, Concrete Sidewalk, and pavement markings on the existing roadways and parking lots in DSCC. Rainstorm drainage slope of the pavement shall be repaired at all areas subject to cold milling and resurfacing by overlaying additional asphalt to achieve positive drain to adjacent catch basins. Details of the Scope of Work and their purpose are described as below: Scope: The Contractor shall furnish all labor, material, supplies, equipment, tools, delivery, disposal and supervision required in performing all operations in conjunction with this project complete, in strict accordance with these specifications and Contract Drawing No. 5-421 subject to the terms and provisions of this contract. This section of the specifications is applicable to all other sections herein. General Crack Filling Maintenance: Center-wide preventive pavement maintenance to be performed on newly developed pavement cracks on "Fair" or "Good" condition pavement based on ASTM PCI (Pavement Condition Index). Areas with 2-inch Mill and Overlay: 10 th Blvd - Repair damaged pavement and poor drainage 17 th Blvd - Repair damaged pavement and poor drainage Foyle Ave - Repair damaged pavement and poor drainage Areas with Full depth pavement repair and other utility repair works: Foyle Avenue North - Foyle North is a major artery connects James Road Gate (Truck Gate) and Ohio National Guard RTIC and Armed Forces Training Center. Many alligator cracks and rutting are identified due to its poor structural capacity and underdrains. The pavement was originally designed for a small street between the warehouses which had been demolished. The existing storm pipes are also identified with many internal damages including complete collapsing, longitudinal and circular cracks during 2009/2010 inspection. New Constructions and Improvements (Capital Minor Construction) - Areas where require new pavements: Foyle Avenue - In addition to pavement and storm drainage repairs, new sidewalk will be constructed along the northeast side and southwest side of the roadway to accommodate increased pedestrian traffic. This section of the roadway, there is no sidewalk for foot traffic on either side of the roadway, and it has been identified as unsafe are for pedestrians. 10 th Blvd - In addition to pavement and storm drainage repairs, existing parking area pavements adjacent to 10 th Blvd will be widened along the west side of the roadway to accommodate increased semi-truck delivery. EVALUATION FACTORS: The following significant Evaluation Factors will be used as the basis for determining Technical Acceptability: Past Performance; Technical Construction Capability; and Technical Qualifications of Proposed Staffing. NOTE: There is a realistic possibility that some offerors may choose to submit the same construction project as a qualifying example for both the past performance and the technical experience evaluation factors. This is not prohibited, but the supporting documentation must demonstrate that the submitted projects absolutely do comply with the specific minimal requirements, identified below, for each separate evaluation factor. To be Technically Acceptable, Offerors should identify no less than THREE (3), and no more than FIVE (5), previously-awarded, and completed construction contracts, which were issued by either: Federal, State, or Local Government Agencies; and/or Private Organizations. To be evaluated as Relevant Past Performance, the construction contracts submitted by offerors must involve: ● Construction work as a prime contractor; ● A dollar magnitude equal to, or greater than $100,000.00; ● Work that was 100% completed within a time period of no more than THREE (3) years prior to the date of submitting this proposal. If the work is still incomplete, or was completed more than three years prior to the date of submitting this proposal, it will not be deemed relevant. For each of the submitted Relevant Past Performance contracts, provide the following information: (a) The name of the construction contract, and the contract ID number; (b) The name of the contracting agency or organization the contract was awarded by; (c) The date of the construction contract award and the contract award amount in dollars; (d) The date of the construction contract completion, and the final contract amount, in dollars; (e) A brief description of the construction contract scope of work; and (f) Identification of at least one point-of-contact at the organization that awarded the submitted contract, including: the name of the point-of-contact; their current mailing address; phone number; and email address (if applicable). (g) If applicable per FAR 15.302(a)(2)(ii) provide information on problems encountered on the submitted contracts and include the corrective actions Note: In accordance with FAR Part 15.305(a)(2)(iv), in the case of an offeror without a record of relevant past performance or for whom information on past performance is not available, the offeror may not be evaluated favorably or unfavorably on past performance. Therefore, the offeror shall be determined to have unknown past performance. In the context of acceptability/unacceptability, "unknown" shall be considered "acceptable" (DOD Evaluation Guide, Section 3.1.3.3.). Such offerors may submit relevant past performance information regarding: predecessor companies; key personnel who have relevant experience; or subcontractors that will be performing major or critical aspects of this requirement; provided that such information is relevant to the construction in this solicitation. Offerors may provide information on problems encountered on the identifiedcontracts and the offeror's corrective action, which may be considered by the Government. The Government also reserves the right to consider past performance information from all available sources, in addition to any information submitted by offerors. FACTOR 2: TECHNICAL CONSTRUCTION CAPABILITY. The term "technical," as used herein, refers to non-price factors other than past performance. More than one "technical" factor can be used and titled to match the specific evaluation criteria appropriate for the RFP. The purpose of the technical factor is to assess whether the offeror's proposal will satisfy the Government's minimum requirements. To be Technically Acceptable, offerors must provide a written technical construction management plan shall be submitted by the offeror to demonstrate their understanding and capabilities of completing all work in accordance with the Statement of Work and project drawings. The plan must identify all work that can be accomplished with in-house resources and demonstrate that they are capable of obtaining any project requirements that cannot be self-performed. The technical construction management plan must identify how the offeror will directly superintend the work, perform project and quality control management, comply with safety and environmental regulations; any project requirements that cannot be self-performed must be identified (e.g. subcontractors list and manufacturers of key material components for the project) The offeror shall provide this information with their company proposal submission. FACTOR 3: TECHNICAL QUALIFICATIONS OF PROPOSED STAFFING. The evaluation team shall evaluate the offeror's proposal against the minimum requirements established by the RFP to determine whether the proposal is acceptable or unacceptable. To be Technically Acceptable, qualified offerors must provide information regarding the technical qualifications of the proposed project staff, to include the individuals who will be responsible for the following functions: Technical Qualifications of Proposed Staffing: Written qualification statement for the individual who will be responsible for project management of the project. Qualifications The Project Manager must have a minimum of 5 years experience as a Project Manager or Superintendent on projects of similar size and complexity. Written qualifications statement for the individual who will be responsible for quality control in accordance with General Requirements Section 01 45 0010 20 Paragraph 1.6.1. Qualifications An individual with a minimum of 5 years combined experience as a superintendent, inspector, QC Manager, project manager, or construction manager on similar size and type construction contracts which included the major trades that are part of this contract. The individual must be familiar with the requirements of the EM 385-1-1 and have experience in the areas of hazard identification and safety compliance. Construction Quality Management Training In addition to the above experience and education requirements, the QC Manager shall have completed the course Construction Quality Management for Contractors and will have a current certificate. Written qualifications statement for the individual who will be responsible for safety in accordance with FAR 52.236-13 Accident Prevention (Nov 1991). Accident Prevention includes compliance with the standards issued by the Secretary of Labor at 29 CFR Part 1926 and 29 CFR Part 1910; and compliance with all pertinent provisions of the latest version of U.S. Army Corps of Engineers Safety and Health Requirements Manual, EM 385-1-1, in effect on the date of the solicitation. This individual will be designated as the Site Safety and Health Officer (SSHO). Qualifications and Training The SSHO(s), as a minimum, must have completed the 30-hour OSHA Construction safety class or as an equivalent, 30 hours of formal construction safety and health training covering the subjects of the OSHA 30-hour course (see Appendix A, paragraph 4.b) applicable to the work to be performed and given by qualified instructors. > The SSHO is also required to have five (5) years of construction industry safety experience or three (3) years if he possesses a Certified Safety Professional (CSP) or safety and health degree. The written qualifications statement shall be certified and signed by a principle of the offeror. Offerors shall submit a resume of each key individual. This will be evaluated to determine if they have the appropriate technical training, certifications, and previous construction experience to meet the technical requirements of a project of this nature. If any of the submitted individuals will perform more than one of the listed functions, this is acceptable as long as the submitted documentation identifies any and all functions the given individual will be performing. •· Third-party construction observation and material inspector shall submit the followings: written qualifications of testing agency's facility, equipment, project manager, and inspector describing work experience as specified in 1.5 and references for projects satisfactorily completed within the last three years, written qualifications of the Contractor demonstrating the contractor is qualified and experienced to perform the specified work within two years. The written qualifications statement shall be certified and signed by the principle of the contracting firm. Offerors need only submit a brief resume of each key individual. This will be evaluated to determine if they have the appropriate technical training, certifications, if required, and previous construction experience to meet the technical requirements of a project of this nature. If any of the submitted individuals will perform more than one of the listed functions, this is acceptable as long as the submitted documentation identifies any and all functions the given individual will be performing. Only qualified tradesmen or mechanics shall be assigned to do the work required by this contract. At no time shall the work be accomplished by apprentices or laborers without the supervision of a journeyman or higher level supervision. Contractor personnel will not require a secret clearance. The contract will be required by law to pay prevailing Service Contract Act wage rates for all work awarded under the contract. The Defense Supply Center Columbus is located in Franklin County, Ohio. PERIOD OF PERFORMANCE: All work associated with the performance of this project must be completed within 180 calendar days after issuance of the Notice to Proceed. Questions concerning this requirement must be submitted no later than 4:00 PM local time on 08 August 2016. The solicitation will close at 1:00 PM (Eastern) on 15 August, 2016.
