Description
The St Cloud Veterans Affairs Health Care System (SCVAHCS), through the Veterans Health Administration Network Contracting Office 23 (VHA NCO 23) is seeking information from Contractors interested in a requirement for one full-time, Independently Licensed Mental Health Professional to furnish Case Management services on or near the Leech Lake Indian Reservation, Cass Lake, Minnesota. This Case Manager will assist with the provision of housing placement services, case management and other supportive services through the Tribal Housing and Urban Development-Veterans Affairs Supportive House (HUD-VASH) Program. The Tribal HUD-VASH Program is designed to bring the HUD-VASH program to American Indian/Native Alaskan tribes living on or near sovereign tribal lands. HUD-VASH is designed to serve homeless Veterans and their families, per the McKinney-Vento Act with the updated definition of "homeless" as found in Homeless Emergency Assistance and Rapid Transition to Housing (HEARTH) Act of 2009. For this Program only, HUD further clarifies and defines "homeless" to incorporate cultural variations to homeless needs specific to tribal locations. The VA intends to engage an adequately credentialed Contractor who possesses the required cultural knowledge and competence to provide case management, supportive and housing placement services to HUD-VASH-eligible Native American Veterans and their families, who are Homeless or At Risk of Homelessness living on or near tribal lands. The unique aspect of bringing the HUD-VASH Program to American Indian/Alaska Native Veterans living on or near sovereign tribal lands requires exclusive knowledge of cultural properties, language, and customs specific to the Leech Lake Band of Ojibwe. In accordance with VHA Handbook 1162.05, HUD-VASH Program, the Case Manager will be a Master's Level Clinician, preferably a Master of Social Work. Case Managers are expected to be able to work independently with professional training at the Master's Level in social work, nursing, other related field, or independently licensed psychologist. The Case Manager is expected to be a licensed independent practitioner. Provisions for Case Manager office space and equipment are to be determined; however, it is likely the Contractor will provide his or her own office space. All Case Management will be done on or near the Leech Lake Indian Reservation, Cass Lake, Minnesota. The VHA NCO 23 intends to award a five (5) year (base plus four (4), one-year options) contract on or before 09/30/16. The total estimated value is within the range of $300,000.00 and $800,000.00. The North American Industry Classification System (NAICS) is 621330 and the size standard is $7.5 million. Any vendor who wishes to identify their interests and capability to provide this service must provide proof of their capabilities as described above, no later than 1:00pm Central, July 26, 2016. Upon receipt of interest and/or capabilities statements (maximum 15 pages in length), the Contracting Officer will determine the acquisition strategy. Responses received will be reviewed and considered; however, a determination by the Government not to compete the proposed procurement, based upon responses to this notice, is solely with the discretion of the Government. This notice is not a request for competitive proposals or quotes. No pricing is requested.
