SOURCES SOUGHT: Request for Capable Offerors THIS SOURCES SOUGHT IS FOR THE HAMPTON VA MEDICAL CENTER IN HAMPTON, VA. The contractor shall provide laboratory support including removable prosthodontics laboratory work for VAMC Hampton. The contractor shall provide all materials and supplies necessary to complete each item. See the requirements included in this Sources Sought Notice. This "Sources Sought Notice" is to determine sources for Hampton VAMC Dental Services with the prescribe restriction listed below in the requirements. This is a SOURCES SOUGHT announcement only. The purpose of this notice is to conduct market research and obtain information from qualified businesses. This is not a request for bids, quotes, or proposals. All responsible sources may submit a capability statement which shall be considered by the agency. The responses to this announcement will assist in determining the socio-economic set aside if feasible. The Veterans Administration is required by Public Law 109-461 to give first priority to SDVOSB and VOSB concerns if there should be sufficient firms available to compete under a set aside. If insufficient veteran owned and small business response is received to establish the basis for setting this action aside for other than large business participation then the action will be advertised as full and open competition, in which case, all interested parties responding shall be eligible to bid. This notice shall not be construed as a commitment by the Government to ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. All information submitted in response to this announcement is voluntary; the Government will not pay for information requested nor will it compensate any respondent for any cost incurred in developing information provided to the Government. This proposed contract is being considered for a set-aside under a small business set-aside program. The North American Industry Classification Systems (NAICS) Code proposed for the requirement is 339116 - Dental Laboratories and the business size standards is 500 employees. The Government is interested in all small businesses. Please indicate if your company is any one or more of the following including: 8(a), Historically Underutilized Business Zone (HUB Zone), Service Disabled Veteran Owned Small Business (SDVOSB), Veteran-Owned Small Business, Woman-Owned Small Businesses (WOSB), Minority-owned Small Business, or any other socio-economic small business status, that are interested in performing this requirement. The Government requests interested parties submit a brief description of their company's capabilities, business size (i.e. annual revenues and employee size), business status (i.e. 8(a), HUB Zone, WOSB, SDVOSB), anticipated teaming arrangements, and a description of similar services offered to the Government and to commercial customers. Any responses involving teaming agreements should delineate between the work that will be accomplished by the prime and the work accomplished by the teaming partners. The Government will use this information in determining its small business set-aside decision. Respondent must be able to meet specifications be able to provide the below requirements. CAPABILITIES STATEMENT: Interested parties must submit a Capabilities Statement, with the cover sheet provided completed, brief and concise, yet clearly demonstrating ability to meet the stated requirements. Interested companies may submit a capabilities statement not to exceed 5 typed pages 8.5x11, to include flyers and miscellaneous advertisements about your company. The Capabilities Statement should clearly present evidence that the interested party feels is relevant. Capabilities Statements are to include the information in the attachment to this sources sought synopsis and title the Subject line for the response: Hampton VAMC CAPABILITIES STATEMENT: Your Company name. IDENTIFY THE VA MEDICAL CENTER(S) FOR THE CAPABILITIES STATEMENT BEING SUBMITTED AND SPECIFY IF YOU ARE INTERESTED IN BOTH LOCATIONS. Please indicate if you currently are registered with GSA, and provide your GSA Schedule information. Interested parties submitting a response to this inquiry must include the following information: name and address of company; business size (large or small); identify applicable socioeconomic categories and any pertinent information which demonstrates firms ability to meet the requirement stated below. All responses shall be submitted no later than July 21, 2016 at 9:00am EST. Responses shall be emailed to: cleveland.wynne@va.gov. Please see the below requirements. Dental Laboratory Services - Hampton VA Medical Center Removable Dentures Hampton, VA A. General Requirement The contractor shall provide laboratory support including removable prosthodontic laboratory work for VAMC Hampton. The contractor shall provide all materials and supplies necessary to complete each item. Acceptable supplies for removable prosthodontics include: Vitallium Alloy, SR- Ivocap High Impact Injectable Resin utilizing SR-Ivocap Injection Molding System and Trubyte IPN or equivalent denture teeth. B. Specific Requirements 1.The contractor shall use procedures, techniques and current technology that are considered to be the standards in the industry. The contractor shall provide items that meet the esthetic and functional requirements of the patient. The contractor shall be familiar with mould guides by Trubyte Bioblend and with shade guides by Trubyte Bioform and the VITA 3D Toothguide. High quality materials shall be utilized from ISO 9001/CE registered companies. Laboratory shall perform internal technical audits to ensure consistent, high quality, and timely restorations. The contractor shall provide a denture storage box for each completed denture case at no additional cost to VAMC Hampton. The contractor shall provide the following dental laboratory services to the VA Medical Center, Hampton, VA: 1.Complete Dentures (including custom trays, bite rims, articulation, set-up, process and finish) 2.Cast Partial Dentures (including custom trays, articulation, frameworks, bite rims, set-up, process and finish) 3.Acrylic Partial Dentures with wrought wire clasps (including articulation) 4.Hybrid Cast/Flexible Partial Dentures (including custom trays, articulation, frameworks, bite rims, set-up, process and finish) 5.Complete and Partial Denture repair (repair of cast partial dentures will be completed using laser welding technique) 6.Complete and Partial Denture relines and rebases 7.Custom Occlusal Guards (including articulation) 8.Valplast Flexible Partial Dentures 9.Surgical stents 10.ID dentures and partials 11.Implant bar overdenture 12.Precision cast retentive bars 13.Precision and semi-precision attachments 14.Fluoride application trays
