PROVIDE SCHOOL BUS SERVICES Work Request #: 6PW140 AAS #: 161660030 Department of the Army US Army Materiel Command (AMC) Tank-Automotive and Armaments Command (TACOM) Watervliet Arsenal (WVA) Installation Management Division (IM) Public Works, Facilities Engineering (IMP-F) 14 June 2016 PART 1 GENERAL A. This Performance Work Statement (PWS) defines the successful performance of providing transportation services, for Army Family Housing (AFH) school students at WVA, to and from local area schools. B. The term of the contract shall be, in general, from 9 September through 30 June according to the school calendar for each school in which transportation services are provided. C. The intent of WVA is to award this contract prior to the start of the 2016-2017 school year, and so the associated 2016-2017 school calendars, as referenced in this PWS, shall be considered specifications of this contract. D. This PWS requires transport services between WVA and the following schools: 1. Shaker Junior High School. 2. Shaker High School. 3. Latham Ridge Elementary School. E. Each route requires the contractor to enter WVA through a security checkpoint where both vehicle and driver are required to display or produce valid identification and certifications. F. All contractors are responsible for complying with DOD, Army, Federal, and State Health and Safety requirements. All work shall be in strict conformance with Watervliet Arsenal Instructions for Contractors (WAIC). The contractor is required to read and understand this document prior to submitting a offer. These instructions are publically available at: 1. http://www.wva.army.mil/Documents/Contracting/WAIC.pdf G. As authorized by WVA, only enrolled eligible Army Family Housing tenant school students, adults serving as chaperones or school personnel shall be transported. The vehicle(s) assigned to the routes specified herein shall not be utilized for other purposes during the time periods designated by the route descriptions. H. Vehicle(s) shall arrive and/or depart the assigned school(s) as indicated on the enclosed route descriptions. I. No transportation contract shall be subcontracted without the prior written approval of WVA. 1.02 VEHICLES A. Transportation equipment shall be properly and currently registered by the NYS Department of Motor Vehicles (DMV), meet all current specifications in accordance with Federal and State law, the rules of the State Board of Education, and any additional specifications of each district's board of education. B. All vehicles shall be systematically inspected in accordance with NYS DMV, and shall bear the proper certification inspection stickers to ensure that such vehicles and equipment are in safe and proper operating condition. C. The contractor shall provide and maintain an adequate number of school buses to safely transport any and all students assigned to the routes contained in this offer and to assure uninterrupted service in the event of mechanical breakdown. 1.03 ACCIDENT REPORTING A. Contractors shall ensure that every school bus driver will: a) immediately inform the proper authorities of the receiving school(s) and the WVA Army Family Housing Manager, following an accident which involves an injury, death, or property damage. Contractors shall properly file accident report with NYS DMV following all applicable guidance and timeframes for filing. 1.04 REPORTING STUDENT LEFT UNATTENDED ON THE SCHOOL VEHICLE A. Every owner/operator of a school vehicle shall immediately inform the administrator or principal of the receiving school(s) and the WVA Housing Manager, or their designee, following an incident in which it has been determined that a student has been left unattended on the school bus at the end of the route. School district or school bus contractor personnel who discover, or to whom it is reported, that a student has been left on a school bus shall immediately report the incident to the owner/operator of the vehicle. B. A student is considered to have been left unattended on the school bus at the end of the route when the driver has left the vicinity of the bus. 1.05 DRIVERS The driver(s) shall be in compliance with valid, current "Article 19-A" Bus driver license in conjunction with the requirements consolidated below. A. Bus drivers operate a motor vehicle, defined as a 'Bus' in the New York State Vehicle and Traffic Law, to transport passengers to pre-planned destinations. B. Bus drivers must pass a qualifying physical examination, and be able to see at least 20/40 in each eye. Bus drivers must demonstrate good eye-hand-foot coordination, and good spatial (three-dimensional) perception to enable good judgment of distance for safe and precise passing, braking and parking. Bus drivers must be able to interact well with both adults and children. C. License Requirements. 1. Bus drivers must possess a Commercial Driver's License (CDL), with a Passenger (P) endorsement (certain school bus drivers also need a School Bus (S) endorsement). The motor carrier company that employs a bus driver must file an application for qualification on behalf of the prospective bus driver with the New York State Department of Motor Vehicles. D. Age Requirements. 1. New York State bus drivers must be 18 years of age. Bus drivers that drive interstate and school bus must be at least 21 years of age. 2. The driver(s) shall be in compliance with the requirements consolidated in the NYS Department of Labor webpage "Bus Driver" located at < http://www.labor.ny.gov/stats/olcny/bus-driver.shtm> E. If, in the judgment of the WVA Housing Manager, any driver or aide assigned to or operating a vehicle under this PWS is deemed to be an unsuitable person for their position because of failure to comply with the rules and regulations, incapacity (physically unable to perform the work), unbecoming conduct, or other good cause, the contractor may be required, at no additional cost to the United States Government (USG), to remove and replace with new, the driver and/or aide from the route. 1.06 EMERGENCY PROVISIONS A. In the event the school is closed due to inclement weather or other emergencies, it shall be the sole responsibility of the Contractor to monitor school closing announcements over local networks. The USG shall not be charged for these dates that transportation does not occur. B. In an emergency where the contractor cannot meet the schedule for any reason, the Contractor shall notify the WVA Housing Manager immediately. 1.07 SUBMITTALS A. Consolidated List of Submittals. Government approval is required for submittals with a "G" designation. Where designated "With Offer" the contractor shall provide the submittal with their offer and prior to award of the contract. The following shall be submitted: 1. Letter from Insurance Company (not Agent) as Proof of Insurance; With Offer 2. Reference List or Evidence Demonstrating the Ability to Perform Required Services; With Offer 3. DMV Operator Licenses; With Offer 4. Facility Information, Include: Facility Location(s), Maintenance Location(s), Office and Dispatch Services, Vehicle Contract Assignments; With Offer 5. Route Map Detailing Required Service Route(s); G 1.08 APPLICABLE DOCUMENTS The following documents are applicable to this PWS and attached appendices. The Contractor shall perform all work in accordance with (IAW) the applicable sections of, but not limited to, the following: A. NYS Department of Labor webpage "Bus Driver" <http://www.labor.ny.gov/stats/olcny/bus-driver.shtm> C. Shaker Junior High School, 2016-2017 School Calendar. D. Shaker High School, 2016-2017 School Calendar. E. Latham Ridge Elementary School, 2016-2017 School Calendar. 1.09 CONTRACT OPTION YEARS A. The contractor shall provide a price with bid for the base year, plus two (2) option years for the performance of this contract. PART 2 PERFORMANCE A. The Contractor shall provide school transport services between WVA and the following schools: 1. Shaker Junior High School. 2. Shaker High School. 3. Latham Ridge Elementary School. Note: Shaker Junior High School and Shaker High School share the same bus route due to having the same school day timeframe of approximately 7:30 AM - 2:17 PM. The intent of this notification is to recommend providing one bus route to serve both Shaker Junior High School and Shaker High School at the same time. In this way, only two (2) individual bus routes are provided to serve three (3) identified schools in the following general manner: For each school day: 1. Bus picks up students at WVA Bus Stop and delivers to Shaker Junior High and Shaker High at the same time (same route). a. ~7:00 AM pickup 2. Bus picks up students at WVA Bus Stop and delivers to Latham Ridge Elementary School. a. ~8:20 AM pickup 3. Bus picks up students at Shaker Junior High and Shaker High at the same time and delivers to WVA Bus Stop. a. ~2:50 PM drop off 4. Bus picks up students at Latham Ridge Elementary School and delivers to WVA Bus Stop. a. ~3:30 PM drop off B. School transport services shall include transport to each school with arrival time at the school in accordance with existing school policy for arrival schedules, generally 15 minutes prior to the start of the first required class of the day. Transportation services at the end of each school day from school to WVA shall be provided in accordance with existing school policy for bus departure schedules. C. School transport services shall be provided on each school day of the full school calendar year as published by each individual school. D. The Contractor shall pick up and drop off students at the two Bus Stops as shown on attachment "Watervliet Arsenal Site Map - School Transportation.pdf" 1. The pick-up and drop off time range shall be specified by the Contractor and relayed to the WVA Housing Manager, whom will relay to the families to whom services are being provided. 2. In the event of special occurrences, e.g. half day of school, state testing, etc. the contractor shall immediately notify the WVA Housing Manager or designee as to the time changes for pick up and drop off. 3. The intent of the service is to provide one (2) individual bus routes, for two specified time ranges, twice a day. E. The contractor shall promptly notify the WVA Housing Manager of any issues or events that are outside of this scope of work, to which said person will provide notice to the Contracting Office for further action, if needed. PART 2 QUALITY ASSURANCE SURVEILLANCE PLAN (QASP) A. SURVEILLANCE PROCEDURES 1. Timeliness: Conformance with existing school schedules, arrival and departure times, ability to adjust timeframes during school emergencies, weather delays or closings. 2. Quality: Conformance with performance work statement and applicable Federal, State and Local standards. 3. Tasks: Performed according to performance work statement requirements. Problem Resolution: Contractor's ability to respond to and successfully resolve issues with the school transportation services as described in this PWS. B. PERFORMANCE AREAS 1. Management Responsiveness: Evaluate the adequacy of the contractor's responsiveness and their ability to provide early problem identification and effective corrective action plans. 2. Schedule Control: Evaluate the contractor's adherence to school schedules 3. Performance: The contractor will be evaluated as to the adherence to the performance work statement and the ability to deliver students to and from school with minimal issues. C. EVALUATION CRITERIA 1. (Outstanding). Indicates performance clearly exceeds contractual requirements. The area of evaluation contains few minor problems for which corrective actions appear highly effective. 2. (Satisfactory). Indicates performance clearly meets contractual requirements. The area of evaluation contains some minor problems for which the corrective actions appear satisfactory. 3. (Unsatisfactory). Indicates the contractor failed to satisfy contractual requirements and recovery is not likely in a timely manner. The area of evaluation contains serious problems for which the corrective actions were ineffective. D. DOCUMENTATION 1. Documentation used to perform surveillance will consist of WVA Housing Manager oversight of the performance of work against this PWS. The WVA Housing Manager will create and keep record of any and all reported issues including late arrivals, no-show occurrences, reports of unbecoming behavior exhibited by the driver(s), reports of inadequate equipment or substandard equipment (school bus is dirty, school bus is unsafe, etc), and vehicle traffic incidents both on and off WVA campus. E. RATING 1. Contractor will be given an overall rating of Outstanding, Satisfactory, or Unsatisfactory by the WVA Housing Manager or Contracting Officer, using the above evaluation criteria.
