Description

I. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice and in accordance with the simplified acquisition procedures authorized in FAR Part 13. This announcement constitutes the only solicitation; proposals/quotes are being requested and a written solicitation will not be issued. All offerors must also be registered on www.sam.gov. Any amendments issued to this solicitation will ONLY be available on the FBO website (www.fbo.gov ). II. This solicitation is issued as a Request for Quotation (RFQ). Submit written quotes on RFQ number VA246-16-Q-1202. This solicitation is a Brand Name or Equal request for Optovue's XR Avanti system with AngioVue; Model Number XR Avanti, to be delivered to the Department of Veterans Affairs (VA) Medical Center at Asheville, North Carolina, which meets the requirements within this solicitation. III. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-87-2. IV. This is a total small business set-aside solicitation. The North American Industry Classification System Code is 339115 and the size standard is 1,000 employees. NOTE: All interested Service Disabled/Veteran Owned Small Business (SDVOSB/VOSB) Offerors must be listed and registered in Vetbiz.gov to be considered for award. V. This combined solicitation/synopsis is for purchase of the following commercial supplies which meet the following salient characteristics and requirements of the brand name system. Any "Or Equal" response shall identify manufacturer and include parts list, along with details on how such item(s) match or exceed the salient characteristics. VI.
