- Dept of Labor Wage Rates Service Contract Information & Clauses Drawings Statement of Work RFQ SF18 Form Pg 2 RFQ SF18 Form Pg 1 Cover Letter SOLICITATION PACKAGE IS ATTACHED TO THIS ANNOUNCEMENT The contractor shall provide all supervision, labor, tools, equipment, transportation and materials required to inspect and test the cathodic protection system for the DF2 and JP5 Fuel Lines at Fuel Pier and Cargo Warf located on the USCG Base Kodiak, Alaska. The contractor shall demonstrate (in writing) an understanding of each of the CP System's design, installation and operating status prior to arrival on -site for inspection and maintenance. The contractor shall be certified by NACE as a Corrosion Protection Specialist, or at a minimum, Corrosion Technician. The contractor must have a minimum of 5 years of experience. The work is located at on the USCG Base, Kodiak, Alaska. Contractor must be IAW SOW. Contractor will be selected based on the price, proposed testing procedures, references and demonstrated understanding of the inspection requirements. All responsible sources that submit a written quotation will be considered. GOVERNMENT FURNISHED MATERIALS: None Performance Period: Provide services within 30 days of notice of proceed. Field work shall be completed by 30 October 2016. SUBMITTALS DUE UPON AWARD may include but are not limited to: Prior to ordering or procuring materials, and a minimum of twenty-one (21) days before start of work, submit three (3) copies of the required information to the Contracting Officer for approval. Include manufacture's catalog data, certificates and other evidence indicating compliance with the specifications Per Submittals Requirements of SOW. Lack of adequacy of references shall be at the sole discretion of the Government and may constitute grounds for disqualification. CONTRACTOR SHALL COMPLY WITH requirements of: applicable federal, state, and local construction and environmental regulations, requirements, best management practices, and standard operation procedures. ADDITIONAL SPECIFICATIONS OR REQUIREMENTS ARE CONTAINED IN STATEMENT OF WORK AND ATTACHED TO THIS ANNOUNCEMENT A SITE VISIT WILL BE HELD Aug 9, 2016 APPROXIMATELY TWO WEEKS BEFORE QUOTE IS DUE. (Site visits are not mandatory, but are highly encouraged.) Location: U.S. Coast Guard Base Kodiak, Alaska. ESTIMATED PRICE RANGE IS LESS THAN $50,000.
