Attachment 1 - Combined synopsis/solicitation Lead and Asbestos Sampling, Analysis, and Inventory in preparation for FY17 Demolish Shuttle Support Facilities, Phase 3. COMBINED SYNOPSIS SOLICITATION NASA/Armstrong Flight Research Center 1.Solicitation Number: NND16584753Q Response Date: not later than 4:00 p.m. (PDT) on August 15, 2016. Description This notice is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. This notice is being issued as a Request for Quotation (RFQ) for the following: Line Item #1- NESHAP SURVEY Quantity: 1each 2.This Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-89 3.Set-Aside Information: Total Small Business NAICS Code: 541620 Environmental Consulting Services Size standard: $15.0M The offeror shall state in their offer their size status for this procurement. All responsible sources may submit a quote which shall be considered by the agency. -------------------------------------------------------------------------------- 02. INSTRUCTION FOR VENDOR -------------------------------------------------------------------------------- The provisions and clauses in the RFQ are those in effect through Current to FAC 2005-89, Effective 14 July, 2016. Quotes for the service(s) described above are due by August 15, 2016 to NASA AFRC via email to alex.j.greenlee@nasa.gov Please provide the following information in your response: 1.Solicitation number, 2.Proposed delivery schedule, 3.Discount/payment terms, warranty duration (if applicable), 4.Commercial and Government Entity (CAGE) code and DUNS Number 5.Identification of any special commercial terms, and be 6.Signed by an authorized company representative. This RFQ is not an order. This is a request for information, and quotations furnished are not offers. This RFQ does not commit the Government to pay any costs incurred in the preparation of the submission of this quotation or to contract for supplies or services. Offerors will be responsible for downloading their own copy of this combination synopsis/solicitation and amendments (if any). -------------------------------------------------------------------------------- 03. ADDITIONAL INSTRUCTIONS TO OFFERORS -------------------------------------------------------------------------------- Offerors shall provide the information required by FAR 52.212-1 Instructions to Offerors-Commercial Items (Oct 2015), which is incorporated by reference. Addenda to FAR 52.212-1 are as follows: 1.Offers must provide: (a)Technical Requirements. i.Offeror shall provide certificate or proof or certification for proposed California certified lead professional(s), Asbestos Consultants and Site Surveillance Technicians who will be performing survey in accordance with the statement of work. (b)Price Offeror shall propose a firm-fixed price for each line item of the solicitation. (c)The Government intends to evaluate offers and award a contract without discussions with offerors. However, the Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. The Government may reject any or all offers if such action is in the public interest; accept other than the lowest offer; and waive informalities and minor irregularities in offers received. 2.Offer must agree with all terms, conditions, and provisions included in the solicitation. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration. 3.The offeror agrees to hold the prices in its offer firm for 90 calendar days from the date specified for receipt of offers, unless another time period is specified in an addendum to the solicitation. 4.Offeror must comply with the requirements of FAR 52.212-1(j) "Data Universal Numbering System (DUNS) Number" and FAR 52.212-1(k) "System for Award Management" to be eligible for award. EVALUATION -------------------------------------------------------------------------------- FAR 52.212-2 Evaluation Commercial Items (Oct 2014) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: 1) Lowest price technically acceptable (LPTA) (i) Technical capability of the item offered to meet the Government requirement (ii) Price The offeror selected for award must submit a completed copy of the provision at 52.212-3 Offeror Representations and Certifications - Commercial Items (Apr 2016) with their offer. The provision may be obtained via the internet at URL: http://farsite.hill.af.mil/reghtml/regs/far2afmcfars/fardfars/far/52_000.htm. These representations and certifications will be incorporated by reference in any resultant contract. -------------------------------------------------------------------------------- 05.
