Description
This "Sources Sought Notice" is to determine sources for Salem VAMC Asbestos Project Monitoring with the prescribe restriction listed below in the Performance Work Statement. This is a SOURCES SOUGHT announcement only. The purpose of this notice is to conduct market research and obtain information from qualified businesses. This is not a request for bids, quotes, or proposals. All responsible sources may submit a capability statement which shall be considered by the agency. The responses to this announcement will assist in determining the socio-economic set aside if feasible. The Veterans Administration is required by Public Law 109-461 to give first priority to SDVOSB and VOSB concerns if there should be sufficient firms available to compete under a set aside. If insufficient veteran owned and small business response is received to establish the basis for setting this action aside for other than large business participation then the action will be advertised as full and open competition, in which case, all interested parties responding shall be eligible to bid. This notice shall not be construed as a commitment by the Government to ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. All information submitted in response to this announcement is voluntary; the Government will not pay for information requested nor will it compensate any respondent for any cost incurred in developing information provided to the Government. Interested parties submitting a response to this inquiry must include the following information: name and address of company; business size (large or small); identify applicable socioeconomic categories and any pertinent information. Performance Work Statement Asbestos Project Monitoring Salem VA Medical Center 1970 Roanoke Blvd Salem, VA 24153 1. GENERAL INTENTION: Consultant shall provide proposal for Asbestos Project Monitoring for the Upgrade Utility Plant project asbestos contaminated soil abatement at the Salem VA Medical Center, in accordance with general items as written. a.Consultant will be requested by the Safety Service as needed during project abatement and invoice after each visit. A visit is determined to be 1 work day on site at the agreed daily rate. b.Coordination. Safety Service will serve as the focal point. c.The Consultant must pre -coordinate work with the Abatement Contractor and General Contractor. This visit will be performed during normal business hours (Monday through Friday). The visit for asbestos project monitoring is anticipated with a shift length of 10 hours. d.The Safety Service will be immediately notified whenever work associated with this project or related activities interfere with normal VAMC operations. 2. STATEMENT OF WORK: This project involves Asbestos Project Monitoring for the Upgrade Utility Plant Project, asbestos contaminated soil abatement, at the Salem VA Medical Center. The Consultants work shall include: a)Asbestos Project Monitoring oversight in accordance with all Federal, State, and Local regulations. Onsite asbestos project monitor shall have a minimum of one year asbestos project monitoring experience and NIOSH 582 certification. b)Review of Abatement Contractor's documentation. c)Regulated area approval once the Abatement Contractor has completed construction. d)Collection of phase contrast microscopy (PCM) air samples outside of the contained work area during abatement. Analysis of PCM samples to be performed onsite. Air sample data to be submitted via email after each visit. e)Final clearance of the work area following abatement, including final visual inspection. f)Final abatement project monitoring report, to include licensure information, daily logs, daily air sample data, final visual inspection, final clearance documentation. Provide electronic copy to the Safety Service within 10 business days of completion of the project monitoring. g)Consultant will check-in with the Safety Service located in Building 74, Room 125. h)NOTE: Work is being completed in a hospital environment. The Consultant must take all precautions to ensure all tools and materials are under continuous observation and accountability. At no time will tools be left unattended or unsecured. i)When clearance has been achieved, notify the Safety Service to inspect and accept all accomplished work. j)Ensure all invoicing documents are submitted in-accordance with the instructions provided in the formal contract. 3. Consultant shall comply in accordance with all OSHA Construction Safety Guidelines. 4. Other Station Rules and Guidelines. The Consultant will adhere to the following station rules. a)All work will be accomplished to industry standards and compliant with all current CODE requirements (Life Safety, NFPA, Electrical, and Building). All Department of Veteran Affairs, Federal regulations, and codes will be complied with. b)This is a hospital environment. The Consultant and his personnel are expected to be appropriately dressed (preferably uniformed) and to conduct themselves professionally, with a minimum amount of noise. c)This is a federal facility as well as a medical center. As such, there is NO tobacco usage allowed inside any federal building or on the grounds except in designated areas. The Safety Service will brief the Consultant as to the designated smoking areas prior to commencement of the contract. Failure to follow these regulations can result in removal of the worker or Consultant from the facility and/or a US Magistrate's ticket and fine. d)Parking will be in designated lots. Consult with the VA police as to allowable parking and material drop-off when you arrive on station. Valet parking is only for patients and should not be utilized. e)The Consultant will responsible for ensuring that all the employees and other personnel visiting the work site have and are using the appropriate Personal Protective Equipment (PPE) that addresses the particular hazards found at the site. The Consultant will be responsible for the upkeep, issue, and training for the PPE used and maintaining all required documentation. Full compliance with OSHA regulations regarding PPE is expected. f)The Consultant shall provide copies of the Safety Data Sheets (SDS) for any chemicals brought onto the Station for this project. g)The Consultant shall provide copies of all training, licensure, medical clearance, fit test records, and a written respiratory protection program at least 10 business days prior to commencing the project. 5. The Consultant shall be solely responsible for the management, professional design, and construction, including all labor, equipment, materials, and inspection, to meet requirements of the contract. 6. The schedule of work is estimated to 1 August to 31 December 2016 (Approx. 40 visits)
