Description
Request for Quotation U.S. Embassy Kabul in Afghanistan would like to Request for Quotation (RFQ) for purchasing of One Forklift per specifications given below. 1.0 PROJECT DESCRIPTION •PROJECT SYNOPSIS The necessity of this Statement of Work (SOW) is for the Contractor to supply one forklift, which will be delivered to the door, and moving EXW. The forklift must meet the criteria as outlined in this SOW. •BACKGROUND C4I, as described on this Statement of Work will require one forklift at the Alvarado ESC Site to meet operational requirements to support RSO Security and Safety Directives. •PROJECT BRIEF This project involves the purchase of one forklift, which will be delivered directly to C4-I at the U.S. Embassy in Kabul, Afghanistan and meet the specifications outlined in the SOW. All work and clearance necessary for the purchase and delivery of the Forklift is the Contractor's responsibility. 2.0 GENERAL CONDITIONS 1. Fixed-Price Proposal. The Contractor shall provide one fixed-priced proposal for the complete project that includes every aspect of the procurement and delivery. 2.Specifications. The procurement shall meet the criteria listed in the SOW. 3.Execution. The purchasing process shall be executed in a diligent and professional manner in accordance with the negotiated fixed-price, this Scope of Work, and the Terms of the Purchase Agreement and all specifications. 4.Visibility. The Contractor will provide weekly status updates tracking the equipment and providing the AWB until final delivery. 5.Safety. The Contractor shall be responsible for insuring a safe delivery of cargo to the U.S. Embassy and that the cargo is delivered damage free, intact, and on time. 6.Modification to Contract. The Contractor shall not incur any costs beyond those described in this SOW unless directed otherwise in writing by the Contracting Officer. Any work performed by the Contractor beyond this SOW without written direction from the Contracting Officer will be at the Contractor's own risk and at no cost to the Embassy. 7.Shipping, Preservation, and Packaging Requirements. The Contractor shall package deliverable hardware such that during transport, the hardware cannot get damaged if subjected to above average road vibrations or rough handling. 8.Close-out. Prior to final acceptance, the Contractor is to submit a detailed list of specifications and line items to insure equipment meets the exact specifications as outlined in the SOW, warranty coverage, delivery timeline, any other relevant detail need to execute purchase and delivery. 3.0 ASSUMPTIONS AND CRITERIA 1.The Contractor is responsible to insure equipment meets specification and all equipment has been provided from a licensed dealer or the OEM. 2.It is a responsibility of the contractor to insure all items as outlined in the SOW are included with the forklift prior to shipping and forwarding a complete packing list to Kabul Procurement. 3.The Contractor is responsible for repairing and or replacing any features that may be damaged on the equipment, if a damage notification is provided by the buyer prior to signing for the equipment on the day of delivery. 4.The buyer has up to 7 calendar days to notify the Contractor of any issues or damage in writing via email or memo. The Contractor will be responsible for repairs or replacement. 5.The Contractor shall also include in his proposal a one-year maintenance contract. 4.0 STATEMENT OF WORK: ACQUISTION AND DELIVERY OF ONE FORKLIFT TO C4- I U.S. EMBASSY, KABUL, AFGHANISTAN The project consists of the basic bid as described below: 5.1 C4I FORKLIFT • Six cylinder 5.8 liter (91 HP) DB58S Diesel Engine Euro Stage or EPA Tier 2 • Auto Shift Transmission • Power Steering • Oil Cooled Disc Brake (ODB) • Electrical fuel shut-off • Anti-restart ignition key switch • Parking Brake • Maintenance free battery • Tilt Steering Wheel. • Dual Element air cleaner and rain cap • Counterweight • Diesel Chassis with oil-cooled brakes • Pneumatic Tire- Single drive • Seating Capacity 2 on 3 of the models, 4 passengers on one model • 3-Section Valve and Lever • Seat-static with hip restraint • Overhead guard • Combination Lamps • Heavy duty radiator • Back up alarm • Side shifter • Tool Kit • Rear view mirrors • Warranty • Standard Mast 2 stage • Lift height 4,000 mm (13.12 feet) • Closed height 3,005 mm • Free Lift 170mm • Carriage Hook-On Side Shift Carriage • Load Backrest 1,331 mm • Pair of forks 60mm x 150mm x 1500mm long • Mast Tilt 8 Degrees FWD/10 Degrees backward 5.0 RESPONSIBILITIES AND PROJECT MANAGEMENT 1.COR. A Contracting Officers Representative (COR) will be assigned to ensure quality assurance goals are met. The Contractor shall provide weekly status updates to the buyer or COR. 2.Point of Contact. The COR shall be the main point of contact for this Project. The Contractor shall report to the COR/C4I on (a) status of the Project, (b) changes in Schedule. 3.Deficiencies and Corrections. The Contractor shall, without additional compensation from the U.S. Embassy, correct any errors or deficiencies in the purchasing and/or other provided services. The Contractor shall make these corrections within 72 hours after being notified of such errors or deficiencies.
