- Updated spreadsheet to reflect updated formulas. The purpose of this amendment is to replace Attachment 2 Pricing Schedule Updated July 22 with Attachment 2 Pricing Schedule Final Updated July 22. This is due to updates with the formulas. Also note: All items required under section IV, #28 Factor I (3) as well as the answer to number 34 on the Question and Answer document, can be submitted as an attachment and will not be included as part of the page limitation. All other terms and conditions remain the same! The purpose of this amendment is to replace Attachment 2 Pricing Schedule Final with Attachment 2 Pricing Schedule Updated July 22. This is due to updates with the formulas. All other terms and conditions remain the same! The purpose of this amendment is as follows: 1.Respond to all the questions. 2.Update solicitation to include changes to the following sections: a.Section IV # 26 b.Section IV # 30 3.Attach updated Statement of Work to include changes to the following: a.Section I.1.9.2 # 1 b.Section I.3.11 4.Attach all the waste streams that are currently available to the Government. 5.Update Pricing Schedule - changes highlighted in blue. 6.Extend due date to August 3, 2016, 11:00 AM ET All other terms and conditions remains the same! NOTE: This is a replacement to solicitation 16-223-SOL-00016 that was originally posted on June 20, 2016. Only the following were amended: 1. Pricing schedule was updated. 2. Page 49 of the solicitation - Pre-poposal Conference - Due date for submitting your questions and names of attendees have been extended to July 6, 2016, 11 AM ET. All other terms and conditions remains the same! The Food and Drug Administration has a continuing requirement to provide all comprehensive waste management services for the collection, transportation and disposal of chemical and radioactive waste generated by the FDA Nationwide, including Puerto Rico
