Description
This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for proposals or quotes and does not obligate the Government to award a contract. Requests for a solicitation will not receive a response. Responses to this Sources Sought must be in writing. The purpose of this Sources Sought Announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Service Disabled Veteran Owned Small Businesses, Veteran Owned Small Businesses, 8(a), Hub-Zone and other Small Businesses interested and capable of providing items requested, as well as any large businesses. Responses to this notice shall include the following: (a) company name b) address (c) point of contact (d) phone, fax, and email (e) DUNS number (f) Cage Code (g) Tax ID Number (h) Type of small business, e.g. Services Disabled Veteran Owned small Business, Veteran-owned small business, 8(a), HUB-Zone, Women Owned Small Business, Small disadvantaged business, or Small Business HUB-Zone business and (i) must provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor for the work described below. Description: The Government is interested in gaining knowledge of potential businesses that would be able to provide radiopharmaceutical deliveries to the VA Health Care System located in Salt Lake City, UT. Statement of Work: STATEMENT OF WORK (SOW) Contractor shall provide and deliver the following radiopharmaceutical supplies, F.O.B. Destination, to the VA Salt Lake City Health Care System 500 Foothill Drive Salt Lake City, UT 84148S The Veterans Affairs Salt Lake City Health Care System (VASLCHCS) Radiology Department is requesting a Base plus 4 option year contract for Radiopharmaceuticals. Radiopharmaceutical Supplies FDG Radiopharmaceutical Dose for PET/CT Studies Technetium Pharmaceuticals Choletec or Hepatolite MAG3 Ultra Tag Kit 99M-Tc DMSA Renal 5 Mci/dose 133-Xe Gas I-131 Therapeutic I-131 Therapeutic Solution I-131 Oral Uptake Solution T1 Thallous Chloride 111-In Indium DTPA Cisternogram 111-In Indium DTPA Gastric 111-In Oxide White Blood Cell 123-I Sodium Iodide Capsules 133-Xe Gas In Saline Tc-99M-HMPAO Brain Tc-99m Sestimibi Tc-99m HPAO Leukocyte In-111 Octreotide 1. All technetium and non-technetium products shall be in stock and available on both a daily and emergency basis as required by VA Salt Lake Nuclear Medicine Service. - All radiopharmaceuticals shall be routinely available for emergency add-ons 7 days a week. - Routine working hours for the VA Nuclear Medicine service are 7:00 a.m. to 4:30 p.m. Monday through Friday - Radiopharmaceuticals ordered the previous day for the scheduled workload shall be delivered no later than 7:00 a.m. the next day. - Emergency Deliveries & Add-on Cases "Normal working hours (7:00 a.m. to 4:30 p.m., Monday through Friday) Supplies must be delivered within 20 minutes of the order being placed with the contractor "Non-working hours (to include weekends and holidays) Supplies must be delivered within 1 hour of the order being placed with the contractor - Contractor will provide 1 free add-on delivery in addition to the daily delivery. Sestimibi and Thallium must be available upon demand. - A current listing of product availability of all radiopharmaceuticals shall be supplied by the vendor. - Ready access to multiple suppliers should be guaranteed to lessen or prevent impact on patient services due to radiopharmaceutical shortage. 2. Contractor shall provide a copy of current licensing with United States Nuclear Regulatory Commission (http://www.nrc.gov/). - The contractor shall remain current with United States Nuclear Regulatory Commission licensing and provide a copy of updated documentation to the Contracting Officer annually. 3. Special services, equipment and expertise for processing special or investigational drugs or synthesis of radiopharmaceuticals which are not commercially available will be provided by the radio pharmacy free of charge. In addition, any specialized processing or synthesis of investigational drugs required for future use will be provided. 4. Chromatography is to be performed on all radiopharmaceuticals for human use before dispensing (including weekend and holiday doses) to ensure labeling efficiency and quality control of preparations. End of statement of work Delivery Address: VA Health Care System 500 Foothill Drive Salt Lake City, UT 84148-0001 The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this sources sought synopsis request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. The North American Classification System (NAICS) code for this acquisition is 339113 (size standard to be considered small business is 500 employees). Notice to Potential Offerors: All Offerors who provide goods or services to the United States Federal Government must be registered in the System Award Management (SAM located on the web at www.sam.gov). It is desirable that any Offeror to have completed their business Online Representations and Certifications Application (ORCA). Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to a solicitation on this project must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at http://vip.vetbiz.gov.
