Picture 3 Picture 2 Picture 1 SF-1449 U.S. ARMY CORPS OF ENGINEERS ST. PAUL DISTRICT ROPE ACCESS BRIDGE INSPECTION SCOPE OF WORK 1. TECHNICAL SCOPE 1.1 General : The Contractor shall provide a rope access inspection team to perform the following tasks: 1.1.2 Perform a combined routine and Fracture Critical Member (FCM) inspection of U.S. Army Corps of Engineers (USACE) lock and dam service bridges over the Mississippi River at: a. Lock and Dam 5A Service Bridge located near Fountain City, Wisconsin. (Approximate length of bridge is 827 feet, Number of Spans: 5-Roller Gates, 5-Tainter Gates, and 3 Storage Yard) b. Lock and Dam 6 Service Bridge located in Trempealeau, Wisconsin. (Approximate length of bridge is 1,078 feet, Number of Spans: 5-Roller Gates, 10-Tainter Gates, and 6 Storage Yard ) c. Lock and Dam 9 Service Bridge located near Eastman, Wisconsin. (Approximate length of bridge is 952 feet, Number of Spans: 5-Roller Gates, 8-Tainter Gates, and 3 Storage Yard) 1.1.3 Perform Fracture Critical Member (FCM)/Routine inspection of four (4) tainter gates: two (2) at Lock and Dam No. 5A and two (2) at Lock and Dam No.6. Government personnel will place bulkheads and raise gates completely out of the water for the purposes of tainter gate inspection. The tainter gates at Locks 5A and 6 are 35 feet wide. 1.1.4 The above listed bridges are on a 60 month inspection cycle in accordance with ER 1110-2-111, USACE Bridge Safety Program, and as such all inspection work shall be completed no later than 30 September, 2016. Inspection duration is estimated at nine (9) total work days. Work will be required on Saturdays. Work will not be conducted on Sundays or Federal Holidays. The commencement of inspection work shall be no earlier than 6 September 2016. The inspection commencement date shall be jointly agreed to between the contractor and the government no later than fifteen calendar days after contract award. 1.1.5 The St. Paul District will supply the inspection program manager who will consolidate all information to include sketches and photos and write the formal bridge inspection report IAW USACE requirements. The inspection program manager will be on site for the entire duration of each inspection. 1.2 Detailed Requirements: The Contractor shall complete the following tasks: 1.2.2 The contractor shall provide an inspection team to perform visual inspections of the bridges and tainter gates at each site. The inspection team shall consist of at least three inspection personnel, the qualified person for fall protection is not required to be one of the three personnel conducting inspection work. The three team members who will be performing inspection work shall have a National Certification in Engineering Technologies (NICET) certification in bridge safety inspection IAW NBIS or equivalent. 1.2.3 In accordance with 23 CFR 650.309 the inspection team shall have one (1) member designated as the Team Leader. The Team Leader must meet all of the qualifications laid out in 23 CFR. An individual who has a Level III certification as a Bridge Safety Inspector under the National Society of Professional Engineer's program for NICET and has completed a Federal Highway Administration (FHWA) approved comprehensive bridge inspection training course would meet the requirements of the CFR. The Team Leader shall be on site during all inspection work. 1.2.4 The contractor shall comply with the requirements of the Corps' Safety and Health Requirements Manual EM 385-1-1. In accordance with EM 385-1-1 one (1) member of the contractor's inspection team shall be a Rope Access Supervisor and / or a person with the training, skills, experience and certification necessary to assume responsibility for the entire rope access work site, including management and guidance of other Rope Access Technicians on the worksite, who is capable of designing, analyzing, evaluating and specifying rope access systems, and who has the knowledge and experience to direct rescue operations from rope access systems, as well as the skills necessary to perform advanced rescue from rope access systems. An individual with a Level III SPRAT or IRATA certification meets these requirements as laid out in EM 385-1-1. A Rope Access Supervisor shall be on site during all inspection work. 1.2.5 The remaining members of the contractor's inspection team shall be Rope Access Workers and / or a person(s) with a professional certificate and appropriate training, skills, and qualifications performing, under the direct supervision of a Rope Access Leader, Technician or Supervisor, standard rope access operations and, at a minimum has the skills necessary to perform rescue from rope access systems. An individual with a minimum Level II SPRAT or IRATA certification meets these requirements as laid out in EM 385-1-1. 1.2.6 The Contractor is responsible for providing their own rope access equipment as well as any additional equipment necessary for a potential rescue scenario. 1.2.7 Record condition of bridge members and tainter gates through the use of St. Paul District forms and digital photographs. Photos must be provided to the inspection program manager no later than 7 calendar days after completion of the inspection via a CD. All photos must contain a date, physical location description, and defect or condition description. 1.3 Limiting Criteria: All training and equipment must meet or exceed OSHA and ANSI requirements, and be in accordance with the current Corps of Engineers Manual EM 385-1-1, entitled Safety and Health Requirements Manual. Contractor personnel shall follow comprehensive safety guidelines for rope access in accordance with EM 385-1-1 (Ex: SPRAT or IRATA) requirements for quality assurance, safety, training, documentation and working practices. U.S. Army Corps of Engineers St. Paul District 180 5th Street East St. Paul, MN 55101 651.290.5242 (Phone) 1.4.3 Field Book, Coding Guide and Forms: All necessary field books, coding guide, and forms will be provided no later than thirty days prior to the scheduled inspections. 1.4.4 Bridge Access. The service bridges at Locks 5A, 6 and 9 are accessible by crossing the miter gates over the main lock chamber and auxiliary lock chamber. Access to the bridge decks is through stairways in the dam piers. The service bridge decks are at least 8 feet wide. All service bridges are equipped with rails to accommodate a crane or lifting carriage Cranes will be locked out by Government personnel each day prior to the start of the inspection. 1.5 Pre Bridge Inspection Submittals: The contractor is required to submit the following information: 1.5.1 Operation Plan. The operation plan shall be a brief paragraph summarizing the understanding of the specified work. This shall be submitted the USACE program manager no later than 15 calendar days prior to the first inspection 1.5.2 Bridge Inspectors Credentials. Copies of each person's credentials that will be participating in the inspection are required along with the contractor's submittal. 1.5.3 Rope access qualifications (Ex: SPRAT or IRATA Credentials). Copies of each person's qualifications including the qualified person for fall protection are required along with the contractor's submittal. 1.5.4 Inspection Tool Kit Description. The contractor shall provide a list of items contained in the inspection tool kit with the contractor's submittal. 1.5.5 Safety Plan. The contractor shall provide a written safety plan to include job hazard analysis for government approval no later than 15 calendar days prior to the first inspection. 1.5.6 Climbing Plan. The contractor shall provide a written climbing plan no later than 15 calendar days prior to the first inspection. The climbing plan may be combined with the safety plan.
