- Attachment #3, Foreign National Access to Whiteman Air Force Base Attachment #2, Schedule with Acknowledgements Attachment #1, Statement of Work This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; Quotations are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ), solicitation number FA4625-16-Q-0079. This procurement is 100% small business set-aside. The North American Industry Classification System (NAICS) code for this acquisition is 238150. For the purpose of this procurement the small business size standard is $15M. All quotes shall be valid for a period of 45 days. The Government intends to issue a single award from this solicitation. Evaluation for this acquisition will be best value based on lowest price technically acceptable. A best value determination will be made by evaluating the following factors: price, and references/past performance. The U.S. Government reserves the right to refrain from awarding to any contractor in the event that all Offerors progressing beyond the technically acceptable evaluation are determined to have offered pricing that is not considered realistic, reasonable, or complete. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-89, Defense Acquisition Circular 20160630, and Air Force Acquisition Circular 2016-0412. Service Labor Standards applies, Department of Labor Wage Decision Number 2005-2307 Revision Number 18, dated 29 December 2015 is hereby incorporated by reference and can be found at the following internet address: http://www.wdol.gov Paper copies of the Request for Quotation will not be made available. Quote shall consist of the completed Attachment #2, Price Schedule & Acknowledgement, and two references of glass sealing or similar services within the last 5 years. The references shall include company name, point of contact and phone number. Quotations can be e-mailed to the following e-mail address: 509cons.sollgca@us.af.mil or by FAX to (660) 687- 4822 to the attention of SrA Deric Ebersohl. NOTE: A site visit for this solicitation will be held on 9 August 2016 at 9:00 a.m. CST. Attendance will be limited to 2 persons per offeror. Meet at the base visitor's center by 9:00 a.m. CST. Email 509cons.sollgca@us.af.mil no later than 8 August 2016 at 9:00 a.m. CST. Provide name, phone number and company name for attendance. A valid Government issued ID will be required for entry, i.e. state issued driver's license. Quotations must be received by 18 August 2016, 2:00 PM CST; late quotations will not be considered for award. Prospective offeror must be registered and have an active status in the System for Award Management (SAM) (www.sam.gov) in order to be eligible for award.
