Request for Quotation Sheet Wage Determination 2005-2339 Statement of Work This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the information in FAR Part 13 using Simplified Acquisition Procedures, as supplemented with the additional information included in this notice. This announcement constitutes the only solicitation; a written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. This combined synopsis/solicitation SHALL be posted on FEDBIZOPPS (www.fbo.gov). The RFQ number is N39040-16-T-0251. This solicitation documents and incorporates provisions and clauses in effect through FAC 2005-89 and DFARS Change Notice 20160630. It is the responsibility of the contractor to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at the following address: http://farsite.hill.af.mil/. The NAICS code is 611430. This will be a 100% Small Business Set-Aside. The size standard for NAICS code 611430 is $11 million. Portsmouth Naval Shipyard Code 410 requests responses from qualified sources capable of providing: A 3-5 day Submarine Design and Operation training class for 15-25 students each year for one base year and two option years. ***See attached RFQ for specifications*** Place of Performance: Off-site training facility - Dell Federal Services Division, 383 Central Ave., Dover, NH 03820. Period of Performance: Training for base period to be conducted between time of award and 30 September, 2016. Exact dates of schedule for base year and option year(s) one and two to be furnished with quote submission. ITEM DESCRIPTION 0001 3-5 Day Submarine Training Design and Operation training for 15-25 students as per attached Statement of Work. POP: Base period to be conducted between time of award and 9/30/2016 1001 Option 1: Day Submarine Training Design and Operation training for 15-25 students as per attached Statement of Work POP: Option 1 to be conducted between 10/1/2016 and 9/30/2017 between August and September timeframes 1002 Option 2: Day Submarine Training Design and Operation training for 15-25 students as per attached Statement of Work. The Government will award a contract resulting from this solicitation to the responsible vendor whose quote conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The contract will be awarded to the vendor with the Lowest Price Technically Acceptable. Quotes will be evaluated based on the following criteria: - Technical Acceptability (Must meet specifications in attachment) - Price Vendors will be required to submit a quote that will be evaluated in accordance with the specifications provided in the solicitation, FAR 52.212-1 Instruction to Offerors. Failure to include pricing for all line items shall be cause for rejection of the quote for all line items. Technical Evaluation Ratings Rating Description Acceptable Submission clearly meets the minimum requirements of the solicitation Unacceptable Submission does not clearly meet the minimum requirements of the solicitation If the technical submittal is determined "Unacceptable", it renders the entire quote technically unacceptable. Technical submissions that do not meet the minimum requirements will be rated as "Unacceptable" and will no longer be considered for further competition or award. System for Award Management (SAM): Vendors must be registered in the SAM database to be considered for award. Registration is free and can be completed on-line at http://www.sam.gov/. METHOD OF PROPOSAL SUBMISSION : All quotes must be sent via email to patricia.perkins@navy.mil All quotes shall include price(s), a point of contact, name and phone number, CAGE Code, technical specifications of units offered (including brand), business size under NAICS Code 611430, and payment terms. At a minimum, quotes shall contain the completed RFQ form (attached). Quotes over 15 pages in total will not be accepted by email.
