Product or Service Code: R426 - SUPPORT-PROFESSIONAL: COMMUNICATIONS Set-Aside: Full & Open NAICS Code: 541519 - OTHER COMPUTER RELATED SERVICES Contracting Office Address: Office of Acquisition Services (OAS) Center for Disease Control and Prevention (CDC) 2920 Brandywine Road Atlanta, GA 30341-5539 Description: All interested companies shall provide proposals for the following per FAR 12.603: (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, "Streamlined Procedures for Evaluation and Solicitation for Commercial Items," as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested, and a separate written solicitation document will not be issued. (ii) This solicitation is a request for proposals (RFP). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-88-1. As seen at: https://www.acquisition.gov/?q=far_looseleaf (iv) This is a Full & Open combined synopsis / solicitation, no Set-Aside, under the associated North American Industrial Classification System (NAICS) code for this procurement is 541519, with a small business size standard of $27.5 million. (vi) The Center for Disease Control and Prevention (CDC) and Agency for Toxic Substances and Disease Registry (ATSDR) Information Technology Services Office (ITSO) located in Atlanta, GA is seeking to purchase Remote Monitoring Services for Microsoft Lync, Exchange and SharePoint without a Virtual Private Network (VPN) or issuance of Government Furnished Equipment (GFE). See attached RFP 2016-N-17953 for further details. (vii) Government Estimated Period of Performance: Transition-In: Date of Award - August 14, 2016 Base Period: August 15, 2016 - July 31, 2017 Option Period 1: August 01, 2017 - July 31, 2018 Option Period 2: August 01, 2018 - July 31, 2019 Option Period 3: August 01, 2019 - July 31, 2020 Option Period 4: August 01, 2020 - July 31, 2021 Transition-Out: July 17, 2021 - July 31, 2021 Place of Performance: The place of performance will be the offerors' place of business via remote, non-VPN, access with site visits to the CDC, ITSO in Atlanta, GA (Roybal and Chamblee campuses).
